摘 要:目的:探究超声心肌分层应变技术对重症心脏病患者左室心功能的评估价值。方法:以2020年6月至2020年12月收治的35例存在心室形态和结构异常的重症心脏病患者为研究组,并以35例同期无明显心室形态和结构异常的重症感染患者为对照组,分析超声心肌分层应变技术指标对心功能的评估价值。结果:研究组左室长轴平均总应变(Global longitudinal strain of average left ventricle,GLS-AVG)、左室二腔观总应变(Global longitudinal strain of left ventricular double-chamber,GLS-A2C)、左室四腔观总应变(Global longitudinal strain of left ventricular four chamber,GLS-A4C)、左室长轴观总应变(Global longitudinal strain of left ventricular long-axis,GLS-LAX)值小于对照组(P<0.05);超声心肌分层应变技术指标联合检测评估患者心功能的AUC大于各指标单独检测,诊断灵敏度、特异度高于各指标单独检测(P<0.05);患者GLS-AVG、GLS-A2C、GLS-A4C、GLS-LAX值随NYHA分级的升高而增大(P<0.05)。结论:超声心肌分层应变技术对重症患者左心室心功能具有较高的评估价值及诊断效能。Objective:To explore the evaluation value of myocardial stratified strain technique for left ventricular function of critical heart disease patients.Methods:A total of 35 critical heart disease patients with abnormal ventricular morphology and structure were enrolled fromJune to December 2020 as study group,while other 35 critical patients without significant abnormalities during the same period were enrolled as control group.The evaluation value of myocardial stratified strain indexes for cardiac function,and their relationship with NYHA grading were analyzed.Results:The global longitudinal strain of average left ventricle(GLS-AVG),global longitudinal strain of left ventricular double-chamber(GLS-A2C),global longitudinal strain of left ventricular four chamber(GLS-A4C)and global longitudinal strain of left ventricular long-axis(GLS-LAX)in study group were lower than those in control group(P<0.05).AUC of myocardial stratified strain technique indexes for evaluating cardiac function was greater than that of single index,and its diagnostic sensitivity and specificity were higher than those of single index(P<0.05).GLS-AVG,GLS-A2C,GLS-A4C and GLS-LAX were increased with the increase of NYHA grading(P<0.05).Conclusion:The myocardial stratified strain technique is of high evaluation value and diagnostic efficiency for left ventricular function in critical patients.
