摘 要:目的:分析低剂量CT检查泌尿系统结石中迭代重建技术(ASIR)对CT图像的影响。方法:选择2019年1-12月厦门大学附属第一医院进行泌尿系统结石诊断治疗的400例患者。使用随机数字法将其分为观察组和对照组,各200例。两组均行CT扫描,对照组常规剂量扫描,观察组低剂量扫描,所有取得图像采用滤过反投影技术(FBP)及ASIR技术进行处理。比较两组在FBP及20%、40%、60%、80%ASIR处理下图像质量评分及图像噪声水平(脂肪、肝右叶、腰大肌),80%ASIR处理下结石位置及大小诊断准确率及辐射剂量。结果:观察组在FBP及20%、40%、60%、80%ASIR处理下图像质量评分均高于对照组(P<0.05)。两组图像在80%ASIR处理下结石位置和大小诊断准确率比较差异均无统计学意义(P>0.05)。观察组CT剂量指数(CTDI)、CT剂量长度乘积(DLP)及有效辐射剂量(ED)均明显低于对照组(P<0.05)。在FBP、20%ASIR、40%ASIR、60%ASIR、80%ASIR处理方法下,观察组脂肪、肝右叶、腰大肌的噪声均高于对照组(P<0.05),噪声随处理方法均呈现依次降低的状态,但观察组80%ASIR条件下的脂肪、肝右叶、腰大肌的噪声与对照组FBP条件下的噪声比较差异无统计学意义(P>0.05)。结论:低剂量CT扫描80%ASIR处理下对CT图像质量影响最小,保证诊断的准确度,同时降低患者所受辐射。Objective:To analyze the influence of adaptive statistical iterative reconstruction(ASIR)on the CT image in low dose CT examination of urinary calculi.Method:A total of 400 patients with urinary calculi diagnosed and treated in the First Affiliated Hospital of Xiamen University from January to December 2019 were selected.They were divided into observation group and control group by random number method,with 200 cases in each group.CT scanning was performed in both groups,conventional dose scanning was performed in the control group and low-dose scanning was performed in the observation group.All obtained images were processed by filtered back projection(FBP)and ASIR technology.The image quality score and image noise level(fat,right lobe of liver and psoas major)under FBP and 20%,40%,60%and 80%ASIR treatment,the diagnostic accuracy of stone location and size under 80%ASIR treatment and radiation dose were compared between the two groups.Result:The image quality scores of the observation group under FBP and 20%,40%,60%and 80%ASIR treatment were higher than those of the control group(P<0.05).There were no significant differences in the diagnostic accuracy of stone location and size between the two groups under 80%ASIR treatment(P>0.05).CT dose index(CTDI),CT dose length product(DLP)and effective radiation dose(ED)in the observation group were significantly lower than those in the control group(P<0.05).Under the treatment of FBP,20%,40%,60%and 80%ASIR,the noise of fat,right lobe of liver and psoas major in the observation group were higher than those in the control group(P<0.05).The noise decreased in turn with the treatment method,but there was no statistical significance in the noise of fat,right lobe of liver and psoas major in the observation group under 80%ASIR condition compared with that in the control group under FBP condition(P>0.05).Conclusion:Low-dose CT scan with 80%ASIR treatment has the least impact on CT image quality,and can ensures diagnostic accuracy and reduces radiation exposure of patients.
