摘  要:目的:了解近5年广安市人民医院临床分离细菌分布及对常用抗菌药物耐药情况,为临床合理选择抗菌药物提供依据。方法:收集广安市人民医院2016-2020年临床分离菌株,统计主要细菌的检出率、耐药性。结果:近5年本院临床分离细菌14885株,其中革兰阴性菌10871株(占73.0%),以流感嗜血杆菌、大肠埃希菌和肺炎克雷伯菌为主。革兰阳性菌4014株(占27.0%),以金黄色葡萄球菌、肺炎链球菌及凝固酶阴性葡萄球菌为主。氨苄西林耐药的流感嗜血杆菌检出率逐年增高。肺炎克雷伯菌对厄他培南和亚胺培南的耐药率升高明显。铜绿假单胞菌对亚胺培南和美罗培南的耐药率升高明显。未发现万古霉素、利奈唑胺耐药的葡萄球菌、屎肠球菌和粪肠球菌。结论:碳青霉烯耐药肺炎克雷伯菌、碳青霉烯耐药铜绿假单胞菌检出率增高明显,需加强感控管理。Objective:To investigate the distribution of pathogenic bacteria in Guang’an People’s Hospital and the resistance of bacteria to commonly used antibacterial agents in recent 5 years,and to provide evidence for clinicians to choose antibiotics reasonably.Method:All the bacterial strains were collected from Guang’an People’s Hospital during the period from 2016 to 2020.The detection rate and antibiotic resistance of main bacteria were analyzed.Result:The total strains detected in our hospital in recent 5 years were 14885.There were 10871 gram-negative bacteria(73.0%),Haemophilus influenza,Escherichia coli,and Klebsiella pneumoniae were the main strains.There were 4014 Gram-positive strains(27.0%),the most common Gram-positive isolates were Staphylococcus aureus,Streptococcus pneumoniae,and coagulase-negative Staphylococcus.The detection rate of Ampicillin resistant Haemophilus influenzae was increased year by year.The drug resistance rate of Klebsiella pneumoniae to Ertapenem and Imipenem increased significantly.The drug resistance rate of Pseudomonas aeruginosa to Imipenem and Meropenem increased obviously.Vancomycin,Linezolid-resistant Staphylococcus,Enterococcus faecium and Enterococcus faecalis were not found.Conclusion:The detection rate of carbapenem-resistant Klebsiella pneumoniae and carbapenem-resistant Pseudomonas aeruginosa increased significantly,and the infection control and management should be strengthened.
