外敷消栓散在出血性疾病患者PICC相关性静脉血栓中的临床应用

摘　　要：目的:探讨外敷消栓散在出血性疾病患者经外周静脉置入中心静脉导管(PICC)相关性静脉血栓(peripherally inserted central catheter-related venous thrombosis,PICC-RVT)中的临床应用。方法:对2018年1月-2021年5月烟台毓璜顶医院中医中西医结合科病房出现过6例出血性疾病患者发生PICC-RVT的案例贴敷自制的消栓散,所用药物为刘寄奴10 g,蒲公英30 g,桃仁15 g,川芎15 g,红花15 g,赤芍15 g,黄芩10 g,贴敷4 h/次,1次/d,中药撤下后用温水擦洗干净皮肤。结果:6例患者分别经过11、22、28、29、34、42 d的贴敷,患有静脉血栓的手臂肿胀、疼痛消失,颜色恢复正常,臂围恢复到置管前的水平,B超显示:患有血栓的静脉最短11 d、最长42 d血流通畅,血栓消失,导管成功再通。结论:消栓散贴敷能有效治疗出血性疾病患者PICC-RVT,值得在临床上推广使用。Objective:To investigate the clinical application of external application Xiaoshuan Powder in peripherally inserted central catheter(PICC)-related venous thrombosis(PICC-RVT)of patients with hemorrhagic disease.Method:Six cases of PICC-RVT in patients with hemorrhagic diseases in ward of integrated traditional Chinese and western medicine department of Yantai Yuhuangding Hospital from January 2018 to May 2021 were applied with self-made Xiaoshuan Powder.The used drugs were Liujinu 10 g,Pugongying 30 g,Taoren 15 g,Chuanqiong 15 g,Honghua 15 g,Chishao 15 g,Huangqin 10 g,applied for 4 h/time,once a day.After the withdrawal of Chinese medicine,the skin was cleaned with warm water.Result:Six patients were applied for 11,22,28,29,34 and 42 days respectively,the swelling and pain of the arm with venous thrombosis disappeared,the color returned to normal,and the arm circumference returned to the level before indwelling catheter.B-ultrasound showed that the blood flow of the vein with thrombosis was unobstructed for the shortest 11 days and the longest 42 days,the thrombosis disappeared,and the catheter was successfully reopened.Conclusion:The attach of Xiaoshuan Powder can effectively treat PICC-RVT in patients with hemorrhagic diseases,which is worthy of clinical application.
