宫腔镜手术联合术后孕激素治疗对子宫内膜息肉患者子宫内膜厚度、月经状况及复发率的影响

摘　　要：目的:探讨宫腔镜手术联合术后孕激素治疗对子宫内膜息肉患者子宫内膜厚度、月经状况及复发率的影响。方法:选取深圳市龙岗区妇幼保健院2018年10月-2020年9月收治的240例子宫内膜息肉患者。按照随机数字表法将其分成观察组(n=120)和对照组(n=120)。两组均采用宫腔镜下子宫内膜息肉切除手术,观察组术后给予地屈孕酮治疗。比较两组术前及术后3、6个月的子宫内膜厚度、月经量、月经期及复发率。结果:术后3、6个月,两组子宫内膜厚度均较术前明显降低,月经量均较术前明显减少(P<0.05),月经期均较术前缩短(P<0.05)。术后3、6个月,观察组子宫内膜厚度均显著低于对照组(P<0.05),月经量均少于对照组(P<0.05),月经期均短于对照组(P<0.05)。观察组术后6个月复发率明显低于对照组(P<0.05)。结论:子宫内膜息肉患者采用宫腔镜手术联合术后孕激素治疗有显著效果,可缓解患者的临床症状,降低复发率,值得临床推广。Objective:To explore the effect of hysteroscopic surgery combined with postoperative progesterone therapy on endometrial thickness,menstrual status and recurrence rate of patients with endometrial polyps.Method:A total of 240 patients with endometrial polyps admitted to Shenzhen Longgang Maternal and Child Health Hospital from October 2018 to September 2020 were selected.According to random number table method,they were divided into observation group(n=120)and control group(n=120).Both groups were treated with hysteroscopic endometrial polypectomy,and the observation group was given Dydrogesterone treatment after operation.Endometrial thickness,menstrual volume,menstruation before operation and 3 and 6 months after operation and recurrence rate were compared between the two groups.Result:Three and six months after operation,endometrial thickness,menstrual volume in both groups were significantly reduced compared with those before operation,and menstruation in both groups was shortened compared with that before operation(P<0.05).Three and six months after operation,the endometrial thickness in the observation group was significantly lower than that in the control group(P<0.05),the menstrual volume was less than that in the control group(P<0.05),and the menstruation was shorter than that in the control group(P<0.05).Six months after operation,the recurrence rate of observation group was significantly lower than that of control group(P<0.05).Conclusion:Hysteroscopic surgery combined with postoperative progesterone therapy for patients with endometrial polyps has significant effects,which can alleviate the clinical symptoms of patients and reduce the recurrence rate.It is worthy of clinical promotion.
