摘 要:目的:探讨连续无创动脉血压(CNAP)监测系统与无创袖带动脉血压(NCAP)监测系统在腰硬联合麻醉(CSEA)后剖宫产监测血压效果的对比。方法:选择2021年1-6月在厦门大学附属妇女儿童医院行CSEA剖宫产手术的106例孕妇,依据数字表法随机分为对照组(n=53,麻醉手术期间行NCAP监测系统监测血压)和观察组(n=53,麻醉手术期间行CNAP监测系统监测血压)。比较两组低血压发生率、血流动力学、血管活性药物使用情况。结果:观察组低血压发生率为77.36%,高于对照组的43.40%(P<0.05);观察组严重低血压发生率为9.43%,低于对照组的26.42%(P<0.05);观察组监测低血压出现时间早于对照组,血流动力学平稳的时间长于对照组,去氧肾上腺素使用剂量少于对照组,差异均有统计学意义(P<0.05)。结论:CNAP监测系统对于CSEA后剖宫产监测具有理想效果,有助于减少低血压及严重低血压的发生率,维持产妇血流动力学稳定,同时减少血管活性药物的使用量,值得推广应用。Objective:To investigate the comparative study of the effect of continuous non-invasive arterial pressure(CNAP)monitoring system and non-invasive cuff arterial pressure(NCAP)monitoring system on monitoring blood pressure in cesarean section under combined spinal-epidural anesthesia(CSEA).Method:A total of 106 cases of pregnant women who underwent cesarean section with CSEA from January to June 2021 in Women and Children’s Hospital Affiliated to Xiamen University were divided into control group(n=53,NCAP monitoring system was used to monitor blood pressure during anesthesia operation)and observation group(n=53,CNAP monitoring system was used to monitor blood pressure during anesthesia operation)according to the random number table method.The incidence of hypotension,hemodynamics and use of vascular active drugs were compared between the two groups.Result:The incidence of hypotension in the observation group was 77.36%,which was higher than 43.40%in the control group(P<0.05).The incidence of severe hypotension in the observation group was 9.43%,which was lower than 26.42%in the control group(P<0.05).The time of hypotensive monitoring in the observation group was earlier than that in the control group,the time of hemodynamic stability in the observation group was longer than that in the control group,and the doses of Phenylephrine Hydrochloride in the observation group were less than those in the control group,the differences were statistically significant(P<0.05).Conclusion:CNAP monitoring system has an ideal effect for cesarean section monitoring with CSEA,which is helpful to reduce the incidence of hypotension and severe hypotension.It can maintains the stability of maternal hemodynamics and reduces the amount of vascular active drugs,which is worthy of promotion and application.
