中老年大疱性类天疱疮与湿疹临床特征及BP180抗体结果分析

摘　　要：目的探讨中老年大疱性类天疱疮(bullous pemphigoid,BP)患者与湿疹患者的临床特征,分析BP180抗体检测对两种疾病的临床意义。方法回顾性分析安徽医科大学第一附属医院2021年1—6月收治的40例中老年BP患者及40例湿疹患者的基本信息、病程、临床表现、伴发疾病、血清BP180抗体指数等临床资料并进行比较。结果中老年BP患者和湿疹患者临床皮损表现不典型,BP组BP180抗体指数显著高于湿疹组(P<0.05),血清BP180抗体指数与BP病情严重程度有相关性,BP组和湿疹组最常见伴发疾病分别为神经系统疾病(27.50%)和心血管系统疾病(27.50%)。结论中老年BP患者合并神经系统疾病多见,早期皮损不典型,易和湿疹混淆;BP180抗体检测对于早期中老年BP和湿疹的鉴别诊断有一定的意义,可作为BP病情评估的指标。Objective To study the clinical features of middle aged and elderly patients with bullous pemphigoid and eczema;analyze the clinical significance of BP180 antibody detection to the two diseases.Method Conduct retrospective analysis of the basic information,course of disease,clinical manifestations,concomitant diseases,and serum BP180 antibody index of 40 BP cases and 40 eczema cases in the First Affiliated Hospital of Anhui Medical University.Result The clinical skin lesions of middle aged and elderly patients with BP and eczema were atypical.Significant difference was observed between the BP group and the eczema group(P<0.05)in terms of BP180 antibody index.The serum BP180 antibody index depends on the severity of BP.The most common concomitant disease of the BP group and the eczema group were nervous system disease(27.50%)and cardiovascular system disease(27.50%).Conclusion Middle aged and elderly BP patients are more vulnerable to neurological diseases.At early stage of the disease,the early skin lesions are atypical,and could easily be confused with eczema;BP180 antibody detection is of certain significance in the differential diagnosis of early senile bullous pemphigus and eczema,and can serve as an evaluation indicator of BP.
