摘 要:目的分析铜山区医疗机构环境空气消毒监测结果现状,为院内感染控制提供科学依据。方法采用卡方检验方法,对2018—2020年监测的铜山区医疗机构环境空气样品合格率进行统计分析。结果共监测空气样品256份,合格198份,合格率77.34%,2018—2020空气合格率分别为67.06%、73.17%、91.01%,合格率差异有统计学意义(χ^(2)=15.437,P<0.001);Ⅰ、Ⅱ、Ⅲ类环境空气合格率分别为86.12%、81.06%、68.19%,合格率差异有统计学意义(χ^(2)=6.835,P=0.034),其中Ⅰ和Ⅲ类、Ⅱ和Ⅲ类环境合格率差异均有统计学意义,分别为(χ^(2)=4.318,P=0.037)和(χ^(2)=4.789,P=0.025);一、二级医疗机构的空气合格率分别为73.53%、92.31%,合格率差异有统计学意义(χ^(2)=27.935,P<0.001);三种消毒方式的空气合格率分别为85.00%、91.03%、67.39%,合格率差异有统计学意义(χ^(2)=17.475,P<0.001),其中空气层流和紫外线灯照射比较,循环风和紫外线灯照射比较,合格率差异均有统计学意义,分别为(χ^(2)=4.703,P=0.037),和(χ^(2)=15.234,P<0.001)。结论铜山区医疗机构空气消毒效果整体合格率相对偏低,尤其一级医疗机构,有明显的管理缺陷,存在一定程度的院感风险。今后要加大消毒投入,规范消毒制度,加强人员培训和消毒监督。Objective To analyze the monitoring results of environmental air disinfection in medical institutions in Tongshan District,so as to provide scientific evidence for nosocomial infection control.Methods Chi-square test was used to analyze the qualified rate of ambient air samples in Tongshan District from 2018 to 2020.Results A total of 256 air samples were monitored,of which 198 were qualified,with a qualified rate of 77.34%.The qualified rates from 2018 to 2020 were 67.06%,73.17% and 91.01%,respectively,and the difference of qualified rates was statistically significant(χ^(2)=15.437,P < 0.001).The qualified rates of class Ⅰ,Ⅱ and Ⅲ were 86.12%,81.06% and 68.19%,respectively,and the difference was statistically significant(χ^(2)= 6.835,P = 0.034).There were statistically significant differences in the qualified rates between class Ⅰ and Ⅲ,and between class Ⅱ and Ⅲ,respectively,were(χ^(2)= 4.318,P = 0.037) and(χ^(2)= 4.789,P = 0.025).The qualified rates of primary and secondary medical institutions were 73.53% and 92.31%,respectively,with statistical significance(χ^(2)= 27.935,P < 0.001).The qualified rates of air of the three disinfection methods were 85.00%,91.03% and67.39% respectively,and the difference of qualified rates among them was statistically significant(χ^(2)= 17.475,P < 0.001).The difference of qualified rates between air laminar flow irradiation and ultraviolet lamp irradiation,and between circulating air irradiation and ultraviolet lamp irradiation were statistically significant,respectively(χ^(2)= 4.703,P = 0.037),and(χ^(2)=15.234,P < 0.001).Conclusion The overall qualified rate of air disinfection effect in medical institutions in Tongshan District is relatively low,especially in first-level medical institutions,there are obvious management defects,and there is a certain degree of risk of hospital infection.In the future,we should increase disinfection input,standardize disinfection system,and strengthen personnel training and disinfection supervision.
