吻合器痔上直肠黏膜环切术在痔疮治疗中的效果

摘　　要：目的探讨痔疮行吻合器痔上直肠黏膜环切术(PPH)治疗的临床效果,为临床治疗痔疮探索更有效的手术方式。方法选取2016年1月—2021年1月天津市公安医院收治的痔疮患者120例为研究对象,随机分为对照组和观察组各60例。对照组采用外剥内扎术,观察组采用PPH术,比较两组治疗时间、疼痛评分、并发症发生率。结果观察组手术、住院、创面愈合、复工等用时为(13.34±2.37)min、(7.63±1.42)、(4.15±2.01)和(7.15±1.05)d,均短于对照组的(29.63±2.52)min、(16.15±2.08)、(8.15±1.07)和(23.25±3.19)d,差异均有统计学意义(P<0.05)。观察组并发症发生率为6.67%,低于对照组的33.33%,差异均有统计学意义(P<0.05)。两组术后24 h疼痛评分差异无统计学意义(P>0.05);术后1周、1个月后,观察组疼痛评分均低于对照组,差异均有统计学意义(P<0.05)。结论对痔疮患者行PPH疗法,能够有效缩短患者治疗时间,降低患者疼痛评分和并发症发生率,值得临床推广应用。Objective To explore the clinical effect of procedure for prolapse and hemorrhoids(PPH) with stapler for hemorrhoids,and to explore the more effective surgical method for the clinical treatment of hemorrhoids.Methods A total of 120 patients with hemorrhoids treated in Tianjin Public Security Hospital from January 2016 to January 2021 were selected as the research object,and randomly divided into control group and observation group with 60 cases in each group.The control group was treated with external stripping and internal ligation,and the observation group was treated with PPH.The treatment time,pain score and incidence of complications were compared between the two groups.Results The time of operation,hospitalization,wound healing and return to work in the observation group were(13.34±2.37) min,(7.63±1.42),(4.15±2.01) and(7.15±1.05) d,which were shorter than(29.63±2.52) min,(16.15±2.08),(8.15±1.07) and(23.25±3.19) d in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 6.67%,which was lower than 33.33% in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in pain score at 24 hours after operation between the two groups(P>0.05).One week and one month after operation,the pain scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion PPH therapy for patients with hemorrhoids could effectively shorten the treatment time,and reduce the pain score and the incidence of complications.It was worthy of clinical promotion and application.
