摘 要:目的研究新疆喀什地区女性高危型人乳头瘤病毒(human papilloma virus,HPV)感染人群X-射线交替互补修复基因1(X-ray alternate complementary repair gene 1,XRCC1)基因多态性与宫颈癌发病的相关性,为宫颈癌防治提供依据。方法选取2019年2月—2021年5月在喀什地区第一人民医院治疗高危型HPV感染宫颈癌患者为研究组,并选取同期体检高危型HPV感染无宫颈癌女性为对照组,使用实时荧光定量聚合酶链式反应(realtime-quantitative polymerase chain reaction,RT-qPCR)法检测两组患者外周血XRCC1-Arg194Trp、Arg280His和Arg399Gln基因型频率分布,并通过二元Logistic回归分析XRCC1-Arg194Trp、Arg280His以及Arg399Gln基因型与HPV感染人群宫颈癌发病的相关性。结果对照组100例患者和研究组100例患者年龄、初潮年龄和产次比较,差异无统计学意义(P>0.05)。研究组和对照组患者XRCC1-Arg194Trp、Arg280His和Arg399Gln基因型频率分布差异均无统计学意义(χ^(2)=2.564和0.635、2.015和0.325、2.742和0.082,P>0.05),两组患者基因型频率分布均符合Hardy-Weinberg平衡定律(P>0.05)。对照组和研究组患者XRCC1-Arg194Trp、Arg399Gln基因型和等位基因分布差异有统计学意义(χ^(2)=5.969和4.298、6.868和4.655,P<0.05),而两组患者XRCC1-Arg280His基因型和等位基因分布差异无统计学意义(χ^(2)=0.302,0.139,P>0.05)。Logistic回归分析携带XRCC1-Arg194Trp和Arg399Gln基因型HPV感染人群罹患宫颈癌的风险分别是未携带人群的1.726倍(OR=1.726)和1.661倍(OR=1.661),而XRCC1-Arg280His与HPV感染人群宫颈癌发病无关(OR=1.235)。结论新疆喀什地区女性XRCC1-Arg194Trp和Arg399Gln基因多态性与HPV感染人群罹患宫颈癌有关,而XRCC1-Arg280His基因多态性与HPV感染人群罹患宫颈癌无关。Objective To study the relationship between the X-ray alternate complementary repair gene 1(XRCC1)gene polymorphism and the incidence of cervical cancer in people infected with high-riskhuman papilloma virus(HPV)in Kashgar,Xinjiang.Methods Patients with high-risk HPV infection with cervical cancer treated in The First People′s Hospital of Kashgar from February 2019 to May 2021 were selected as the study group,and HPV-infected women without cervical cancer who had a physical examination in our hospital during the same period selected as the control group.We used real timequantitative-polymerase chain reaction(RT-qPCR)method to detect the frequency distribution of XRCC1-Arg194Trp,Arg280His and Arg399Gln genotypes in the peripheral blood of the two groups of patients.And the correlation between XRCC1-Arg194Trp,Arg280His and Arg399Gln genotypes and the incidence of cervical cancer in HPV infected people were analyzed using binary Logistic regression.Results There were no significant differences in age,age of menarche and birth rate between 100 patients in the control group and 100 patients in the study group(P>0.05),and the frequency distribution of XRCC1-Arg194Trp(Control group:χ^(2)=0.635;Study group:χ^(2)=2.561),Arg280His(Control group:χ^(2)=0.325;Study group:χ^(2)=2.015)and Arg399Gln(Control group:χ^(2)=0.082;Study group:χ^(2)=2.742)genotypes in the two groups conformed to the Hardy-Weinberg equilibrium law(P>0.05).There were significant differences in xRCC1-ARG194Trp and Arg399Gln allele distribution between the two genotypes of control and study group patients(χ^(2)=5.969 and 4.298,6.868 and 4.655,P<0.05);there was no significant difference between genotype XRCC1-arg280his and allele distribution(χ^(2)=0.302,0.139,P>0.05).Logistic regression analysis showed that the HPV-infected population with XRCC1-Arg194Trp and Arg399Gln genotypes had 1.726 times the risk of cervical cancer(OR=1.726)and 1.661 times(OR=1.661),and XRCC1-Arg280His had nothing to do with the incidence of cervical cancer in HPV infected
