调脂降糖片对肥胖型2型糖尿病痰瘀互结证患者的临床疗效

摘　　要：目的观察调脂降糖片对肥胖型 2 型糖尿病(type 2 diabetes mellitus,T2DM)痰瘀互结证患者的临床疗效。方法采用随机、对照、平行设计方法,纳入于2018年2月至2019年4月上海中医药大学附属龙华医院内分泌科收治的肥胖型T2DM痰瘀互结证患者75例,采用随机数字表法分为治疗组(调脂降糖片+二甲双胍,n=40)及对照组(二甲双胍,n=35),疗程均为12周,观察治疗前后患者临床症状、血糖、胰岛素抵抗、肥胖情况、炎症细胞因子水平的变化。结果两组中医证候评分、空腹胰岛素、餐后2 h胰岛素、稳态模型胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOME-IR)、体质量指数、腰围、白介素-6较治疗前均降低(P<0.05),且治疗组优于对照组(P<0.05)。两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平较治疗前均降低(P<0.05),但两组间差异无统计学意义。治疗组肿瘤坏死因子-α较治疗前降低(P<0.05),对照组较治疗前差异无统计学意义(P>0.05)。治疗组(85%)中医证候总有效率优于对照组(62.86%)(P <0.05)。结论调脂降糖片对T2DM痰瘀互结证患者的症状和体征、胰岛素抵抗、肥胖情况、炎症细胞因子水平等方面有较佳的改善作用,值得临床进一步推广。Objective To observe the clinical effects of Tiaozhi Jiangtang tablets on patients with type 2 diabetes mellitus (T2DM) patients with phlegm and blood stasis syndrome.Methods A total of 75 obesity T2DM patients with phlegm and blood stasis syndrome admitted to the Department of Endocrinology,LongHua Hospital Shanghai University of Traditional Chinese Medicine(TCM) from February 2018 to April 2019 were randomly divided into the treatment group(Tiaozhi Jiangtang tablets+metformin,n=40) and the control group(metformin,n=35).The treatment course was 12 weeks.The changes of clinical symptoms,blood glucose,insulin resistance,obesity and inflammatory cytokines were observed before and after treatment.Results The TCM syndrome score,fasting insulin,2 h postprandial insulin,homeostasis model assessment of insulin resistance(HOME-IR),body mass index,waist circumference and interleukin-6 in the two groups were lower than those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).The levels of fasting blood glucose,2-hour postprandial blood glucose and glycosylated hemoglobin in the two groups were lower than those before treatment(P<0.05),but there was no significant difference between the two groups(P>0.05).Compared with before treatment,tumor necrosis factor-α in the treatment group was decreased(P<0.05),but there was no significant difference in the control group(P>0.05).The total effective rate of TCM syndrome in the treatment group(85%) was better than that in the control group(62.86%)(P<0.05).Conclusion Tiaozhi Jiangtang tablets can improve the symptoms and signs,insulin resistance,obesity and inflammatory cytokine level of obesity T2DM patients with phlegm and blood stasis syndrome,which is worthy of further clinical promotion.
