交锁髓内钉有限切开复位内固定术治疗下肢长骨骨折的效果观察

摘　　要：目的探讨交锁髓内钉有限切开复位内固定术治疗下肢长骨骨折的效果。方法抽取2017年3月至2021年3月济源市人民医院收治的下肢长骨骨折患者64例,依据治疗方案分为髓内钉组和钢板组,每组32例。钢板组予以切开复位钢板内固定术,髓内钉组予以交锁髓内钉有限切开复位内固定术。比较两组围术期指标、骨折愈合率、膝关节功能恢复优良率、视觉模拟量表(VAS)评分、并发症发生率。结果髓内钉组手术时间、骨折愈合时间少于钢板组(t=11.505、2.890,P<0.05)。髓内钉组骨折愈合率为96.88%(31/32),高于钢板组的81.25%(26/32),P<0.05。髓内钉组膝关节功能恢复优良率高于钢板组(χ^(2)=4.730,P=0.030)。术后3个月,髓内钉组VAS评分低于钢板组(t=5.167,P<0.05)。髓内钉组并发症发生率与钢板组比较,χ^(2)=0.267,P=0.606。结论交锁髓内钉有限切开复位内固定术可加快手术进程,促进关节功能恢复,且可减少并发症,减轻疼痛度。Objective To investigate the effects of limited open reduction and internal fixation with interlocking intramedullary nails on long bone fractures of the lower extremities.Methods A total of 64 patients with long bone fractures of lower extremities admitted to Jiyuan People’s Hospital from March 2017 to March 2021 were selected and divided into intramedullary nail group and steel plate group according to the treatment plan,with 32 cases in each group.The steel plate group was treated with open reduction and plate internal fixation,and the intramedullary nail group was treated with limited open reduction and internal fixation with interlocking intramedullary nails.The perioperative indexes,fracture healing rate,knee function recovery rate,visual analog scale(VAS)score,and complication rate were compared between the two groups.Results The operation time and fracture healing time in the intramedullary nail group were less than those in the steel plate group(t=11.505,2.890,P<0.05).The fracture healing rate in the intramedullary nail group was 96.88%(31/32),which was higher than 81.25%(26/32)in the steel plate group,P<0.05.The rate of good knee function recovery in the intramedullary nail group was higher than that in the steel plate group(χ^(2)=4.730,P=0.030).Three months after the operation,the VAS score in the intramedullary nail group was lower than that in the steel plate group(t=5.167,P<0.05).The complication rate in the intramedullary nail group was compared with that in the steel plate group,χ^(2)=0.267,P=0.606.Conclusions The limited open reduction and internal fixation with interlocking intramedullary nails can speed up the operation process,promote the recovery of joint function,and reduce complications and pain.
