阴道三维超声评估宫腔粘连患者宫腔容积和血流变化的价值

摘　　要：目的探讨阴道三维超声评估宫腔粘连患者宫腔容积和血流变化的价值。方法抽取2017年1月至2020年12月河南省生殖妇产医院收治的宫腔粘连患者100例设为宫腔粘连组,抽取同时期体检宫腔正常的受试者100例作为健康对照组。统计两组阴道三维超声对宫腔粘连的诊断率;比较两组宫腔容积和宫腔血流参数。结果宫腔粘连组患者阴道三维超声诊断结果显示,Ⅰ型宫腔粘连23例,Ⅱ型宫腔粘连52例,Ⅲ型宫腔粘连14例,Ⅳ型宫腔粘连5例,6例患者未检出宫腔粘连,阴道三维超声对宫腔粘连的诊断率为94%(94/100)。宫腔粘连组患者宫腔容积低于健康对照组(P<0.05)。宫腔粘连组患者宫腔血流参数如血管化指数(VI)、血流指数(FI)和血管化血流指数(VFI)低于健康对照组(P<0.05)。结论阴道三维超声对宫腔粘连具有较高的诊断率,其能够定量检测宫腔粘连患者的宫腔容积以及宫腔血流变化情况,对宫腔粘连的临床诊断具有重要的参考价值。Objective To explore the value of three-dimensional transvaginal ultrasound in evaluating the changes of uterine volume and blood flow in patients with intrauterine adhesions.Methods A total of 100 cases of intrauterine adhesions admitted to Henan Province Reproduction Maternity Hospital from January 2017 to December 2020 were included retrospectively and set as intrauterine adhesions group.In addition,100 subjects with normal uterine cavity were selected as healthy control group.The diagnostic rates of intrauterine adhesion by three-dimensional transvaginal ultrasound in the two groups were counted;the uterine volume and uterine blood flow parameters were compared between the two groups.Results Three-dimensional vaginal ultrasound diagnosis showed that there were 23 cases of type Ⅰ intrauterine adhesions,52 cases of type Ⅱ intrauterine adhesions,14 cases of type Ⅲ intrauterine adhesions and 5 cases of type Ⅳ intrauterine adhesions.No intrauterine adhesions were detected in 6 patients.The diagnostic rate of three-dimensional vaginal ultrasound diagnosis of intrauterine adhesions was 94%(94/100).The uterine volume in intrauterine adhesions group was lower than that in healthy control group(P<0.05).The values of vascularization index(VI),flow index(FI)and vascularization flow index(VFI)in intrauterine adhesions group were lower than those in healthy control group(P<0.05).Conclusions Vaginal three-dimensional ultrasound has a high diagnostic rate for intrauterine adhesion.It can quantitatively detect the intrauterine volume and the changes of intrauterine blood flow in patients with intrauterine adhesion.It has important reference value for the clinical diagnosis of intrauterine adhesion.
