摘　　要：目的探讨三维彩超在妊娠前期肥胖孕妇胎盘血管监测中的应用。方法选取2019年6月至2021年6月本院产科收治的92例产检妊娠期(孕11~16^(+6)周)孕妇作为研究对象,根据体重指数(BMI)分为超重组(BMI≥24 kg/m^(2))与非超重组(BMI<24 kg/m^(2)),每组46例。两组均行三维多普勒超声检查,比较两组超声监测参数、妊娠相关蛋白-A(PAPP-A)、胎盘蛋白13(PP13)。结果超重组胎盘体积胎盘体积(PV)、血管化指数(VI)、血流指数(FI)、血管化/血流指数(VFI)均低于非超重组(P<0.05),子宫动脉搏动指数(PI)高于非超重组(P<0.05),超重组PAPP-A水平低于非超重组(P<0.05),两组PP13水平比较差异无统计学意义。结论三维彩超能定量分析孕妇的胎盘血流灌注情况,为诊断妊娠前期肥胖孕妇高血压等不良情况提供有效依据。Objective To explore the application of three-dimensional color Doppler ultrasound in placental blood vessels of obese pregnant women in pre-pregnancy.Methods 92 pregnant women during pregnancy(11-16^(+6) weeks gestation)in the obstetrics department of our hospital from June 2019 to June 2021 were selected as the research subjects.They were classified as overweight group(BMI≥24 kg/m^(2))and non-overweight group(BMI<24 kg/m^(2))according to the body mass index(BMI),with 46 cases in each group.Three-dimensional color Doppler ultrasound was performed in both groups,and the ultrasound monitoring parameters,pregnancy-associated protein-A(PAPP-A)and placental protein 13(PP13)were compared between the two groups.Results Placental volume(PV),vascularization index(VI),blood flow index(FI),vascularization/blood flow index(VFI)of overweight group were lower than those of non-overweight group(P<0.05),the uterine artery pulsation index(PI)was higher than non-overweight group(P<0.05),and the level of super-recombination PAPP-A was lower than that of non-overweight group(P<0.05).There was no significant dif-ference in PP13 levels between the two groups.Conclusion Three-dimensional color Doppler ultrasound can quantitatively analyze the placental blood perfusion of pregnant women,and provide an effective basis for the diagnosis of obese pregnant women with high blood pressure and other ad-verse conditions.
