通理汤联合中药熏洗对PPH患者预后及血清VEGF bFGF PDGF水平的影响

摘　　要：目的探讨通理汤联合中药熏洗治疗对吻合器痔上黏膜环形切除患者预后及血清血管内皮生长因子、碱性成纤维生长因子、血小板源性生长因子水平的影响。方法将108例吻合器痔上黏膜环形切除术后患者根据术后治疗方法分为两组,每组54例。西药组给予高锰酸钾熏洗,中药组给予通理汤联合中药熏洗治疗。比较两组临床疗效、并发症发生率,术后当天及术后7 d血清血管内皮生长因子、碱性成纤维生长因子、血小板源性生长因子水平及肛周疼痛、坠胀、水肿、创面渗液评分。结果中药组治疗总有效率显著高于西药组(P<0.05)。术后7 d两组血清血管内皮生长因子、碱性成纤维生长因子、血小板源性生长因子水平均显著高于术后当天(P<0.01),中药组显著高于西药组(P<0.01)。术后7 d两组肛周疼痛、坠胀、水肿、创面渗液评分均显著低于术后当天(P<0.01),中药组显著低于西药组(P<0.01)。中药组并发症发生率显著低于西药组(P<0.05)。结论通理汤联合中药熏洗治疗可上调吻合器痔上黏膜环形切除患者血清血管内皮生长因子、碱性成纤维生长因子、血小板源性生长因子水平,有效缓解肛周疼痛、坠胀、水肿、创面渗液等症状,促进创面愈合,降低并发症发生率。Objective To investigate the effect of Tongli decoction combined with traditional Chinese medicine fumigation and washing on serum vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)and platelet derived growth factor(PDGF)levels and prognosis in patients with procedure for prolapse and hemorrhoids(PPH).Methods 108 patients with PPH were divided into two groups according to the postoperative management method,54 cases in each group.The western medicine group was given potassium permanganate fumigation,and the traditional Chinese medicine group was given Tongli decoction combined with traditional Chinese medicine fumigation and washing.The clinical efficacy,incidence of complications,serum VEGF,bFGF and PDGF levels,and scores of perianal pain,bulge,edema,and wound exudation on the day and 7 days after surgery were compared between the two groups.Results The total effective rate of traditional Chinese medicine group was significantly higher than that of western medicine group(P<0.05).The levels of serum VEGF,bFGF and PDGF in the two groups on the 7th day after operation were significantly higher than those on the day after operation(P<0.01),and the traditional Chinese medicine group was significantly higher than the western medicine group(P<0.01).The scores of perianal pain,bulge,edema and wound exudation in the two groups on the 7th day after operation were significantly lower than those on the day after operation(P<0.01),and the traditional Chinese medicine group was significantly lower than the western medicine group(P<0.01).Conclusions Tongli decoction combined with traditional Chinese medicine fumigation and washing can increase the levels of serum VEGF,bFGF and PDGF in patients with PPH,effectively relieve symptoms such as perianal pain,bulge,edema,and wound exudation,promote wound healing,and reduce the incidence of complications.
