4R危机理论风险评估护理管理在心血管重症监护病房中的应用

摘　　要：目的探讨4R危机理论风险评估护理管理在心血管重症监护病房中的应用效果。方法将94例心血管重症监护病房的患者按入院时间分观察组(49例)、对照组(45例)。对照组予以常规护理管理,观察组在对照组基础上予以4R危机理论风险评估护理管理,观察7 d。比较两组风险事件发生率、护理满意度、护理前后急性生理和慢性健康评分系统评分,比较4R危机理论实施前后心血管重症监护病房护士理论及操作考核评分。结果两组风险事件发生率比较差异无统计学意义(P>0.05)。护理3 d、7 d末两组急性生理和慢性健康评分系统评分均较护理前显著降低(P<0.05),观察组显著低于对照组(P<0.05)。4R危机理论实施后心血管重症监护病房护士理论考核、操作考核得分较实施前显著升高(P<0.05)。观察组护理总满意率显著高于对照组(P<0.05)。结论4R危机理论风险评估护理管理能提高心血管重症监护病房责任护士的专业技能,降低风险事件发生率,促进患者康复,提高护理满意度。Objective To explore the application effect of risk assessment nursing management based on 4R crisis theory in nursing work in coronary care unit(CCU).Methods A total of 94 patients in CCU were divided into observation group(49 cases)and control group(45 cases)according to the time of admission.The control group was given routine nursing management,and the observation group was given 4R crisis theory risk assessment nursing management on the basis of the control group for 7 days.The incidence of risk events,nursing satisfaction and acute physiology chronic health evaluation-Ⅱ(APACHE-Ⅱ)score of patients before and after nursing were compared between the two groups.The theoretical and operational assessment scores of nurses in CCU were compared between the two groups.Results There was no significant difference in the incidence of risk events between the two groups(P>0.05).The APACHE-Ⅱ scores of the two groups at the end of the 3rd and 7th days of nursing were significantly lower than those before nursing(P<0.05),and the observation group was significantly lower than the control group(P<0.05).After the implementation of 4R crisis theory,the scores of nurses'theoretical and operational assessments in CCU were significantly higher than those before nursing(P<0.05).The total nursing satisfaction rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusions The application of risk assessment nursing management based on 4R crisis theory in CCU nursing work is helpful to improve the professional skills of responsible nurses,control the incidence of risk events,promote the rehabilitation of patients,and improve nursing satisfaction.
