氨溴索联合鼻塞式持续气道正压通气对重症新生儿肺炎治疗效果的影响

摘  要:目的:探讨氨溴索联合鼻塞式持续气道正压通气对新生儿肺炎治疗的临床效果。方法:选取2019年3月-2021年11月邳州市人民医院收治的新生儿肺炎患儿60例,随机分为对照组和观察组,各30例。对照组给予抗病毒、抗感染、纠正水电解质紊乱等对症治疗措施,采用鼻塞式持续气道正压通气治疗,持续正压通气。观察组在对照组的基础上增加盐酸氨溴索治疗,7.5 mg加入10 mL葡萄糖注射液中,微泵滴入,2次/d,连续治疗3~5 d。比较两组患儿临床症状消失时间、持续正压通气治疗时间、住院时间、肺功能及动脉血气指标和并发症发生率。结果:观察组患儿症状消失时间、持续正压通气治疗时间、住院时间均明显低于对照组,观察组患儿氧合指数、动脉血氧分压明显高于对照组,观察组动脉血二氧化碳分压明显低于对照组,差异均有统计学意义(P<0.05)。观察组并发症发生率与对照组比较,差异无统计学意义(P>0.05)。结论:氨溴索联合鼻塞式持续气道正压通气治疗重症新生儿肺炎疗效显著,可以明显缓解患儿的症状,增强呼吸功能。Objective:To investigate the clinical effect of ambroxol combined with nasal continuous positive airway pressure ventilation in the treatment of neonatal pneumonia.Methods:A total of 60 children with neonatal pneumonia treated in Pizhou People’s Hospital from March,2019 to November,2021 were selected and randomly divided into control group and observation group,with 30 cases in each group.The control group was treated with anti-virus,anti-infection,correction of water and electrolyte disorders and other symptomatic treatments,and received nasal continuous positive airway pressure ventilation.The observation group was additionally treated with ambroxol hydrochloride on the basis of the control group,7.5 mg was added into 10mL glucose injection and dropped through micropump,twice a day for continuous treatment for 3-5 days.The disappearance time of clinical symptoms,continuous positive pressure ventilation time,length of hospital stay,pulmonary function,arterial blood gas indexes and the incidence of complications were compared between the two groups.Results:The disappearance time of clinical symptoms,continuous positive pressure ventilation time,length of hospital stay in observation group were significantly lower than those in control group,the oxygenation index and arterial partial pressure of oxygen in the observation group were significantly higher than those in the control group,while arterial partial pressure of carbon dioxide in the observation group was significantly lower than those in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in the incidence of complications between two groups(P>0.05).Conclusion:Ambroxol combined with nasal continuous positive airway pressure ventilation in the treatment of severe neonatal pneumonia has significant efficacy,it can relieve the child’s symptoms and enhance pulmonary function.
