摘  要:目的本研究的目的是评估暴露于生物质烟雾和香烟烟雾对慢性呼吸衰竭(CRF)患者血清炎症标志物和肺功能参数的影响。方法142例CRF患者分为年龄和性别匹配的生物质烟雾暴露组68例;香烟烟雾暴露组74例。有关患者一般资料(年龄和性别)、炎症标志物(包括中性粒细胞与淋巴细胞的比值,C-反应蛋白,血小板/平均血小板体积比)、动脉血气分析和肺功能检查结果的数据,包括第1秒用力呼气量(FEV1),用力肺活量(FVC)和FEV1/FVC,从医疗记录中获得。结果生物质烟雾暴露组的二氧化碳分压水平(mm Hg)显著高于香烟烟雾暴露组[(50.5±7.0)mm Hg vs(47.1±7.7)mm Hg],差异有统计学意义(P=0.006)。肺功能测定显示,生物质烟雾和香烟烟雾暴露组差异无统计学意义(P>0.05),FEV1/FVC升高、FVC降低,FEV1/FVC<70%患者的百分比低,差异均有统计学意义(P=0.001)。结论在生物质烟雾暴露组和香烟烟雾暴露组,炎性标志物的增加相似,并且肺功能测试结果异常,强调了生物质烟雾暴露对肺的不利影响与香烟烟雾暴露一样重要。与香烟烟雾接触相比,生物质烟雾接触与更高的FEV1/FVC比值>70%的患者比例以及更严重的肺活量损失相关性更大。Objective The purpose of this study was to evaluate the effects of exposure to biomass smoke and cigarette smoke on serum inflammatory markers and lung function parameters in patients with chronic respiratory failure(CRF).Methods A total of 142 CRF patients were divided into an age and gender-matched biomass smoke exposure group and cigarette smoke exposure.General information about the patients(age and gender),inflammatory markers(including the ratio of neutrophils to lymphocytes,C-reactive protein,platelet/mean platelet volume ratio),arterial blood gas analysis and pulmonary function test results,including data 1 second forced expiratory volume(FEV1),forced vital capacity(FVC)and FEV1/FVC obtained from medical records.Results The carbon dioxide partial pressure level(mm Hg)of the biomass smoke exposure group was significantly higher than that of the cigarette smoke exposure group,with the differences statistically significant[(50.5±7.0)mm Hg vs(47.1±7.7)mm Hg,P=0.006].Pulmonary function tests showed that there was no statistical significance on the difference between biomass smoke and cigarette smoke exposure group(P>0.05).FEV1/FVC increased,while FVC(ml)decreased.The percentage of patients with FEV1/FVC<70%was low,the difference was statistically significant(P=0.001).Conclusion Our findings indicate that in the biomass smoke exposure group and the cigarette smoke exposure group,the increase in inflammatory markers is similar,and the lung function test results are abnormal,indicating that adverse lung effects from biomass smoke exposure are just as important as cigarette smoke exposure.Compared with cigarette smoke exposure,biomass smoke exposure is more associated with a higher proportion of patients with a higher FEV1/FVC ratio>70%and more severe lung capacity loss.
