372例高胆红素新生儿溶血三项检查结果分析

摘  要:目的探讨溶血三项检测在高胆红素新生儿中的临床应用.方法选择连云港市东方医院住院儿科及门诊儿科在2018年2月-2021年2月收治的372例高胆红素血症患儿,对其溶血三项试验结果进行回顾性分析.结果372例患儿中检测阴性303例,可疑2例,确诊新生儿溶血病67例,新生儿溶血病检出率为18.0%,其中,67例确诊患儿中,红细胞抗体释放试验阳性率100%,血清游离抗体试验阳性率62.7%,直接抗人球蛋白试验阳性率1.4%.男性患儿检出率21.2%,女性患儿检出率13.4%,无显著差异(P=0.052>0.05).溶血三项检测结果阳性率与标本送检时间有关,送检时间在两天内(23.32%)和在3天及以上(11.73%)的检出率具有显著差异(P=0.003<0.05).结论溶血三项试验在新生儿溶血病的诊断中具有重要意义,其中释放试验是诊断新生儿溶血病的主要依据,对于临床有症状或者怀疑新生儿溶血病的患儿应尽早检测,最好在两天内进行溶血三项的检测,提高诊断效率.Objective To explore the clinical application of three tests of hemolysis in neonates with high bilirubin.Methods We selected 372 children with hyperbilirubinemia admitted in the inpatient and outpatient pediatrics department of our hospital from February 2018 to February 2021,and retrospectively analyzed the results of the three hemolysis tests.Results of the 372 cases,303 cases were negative and 2 cases were suspicious.67 cases of neonatal hemolytic disease were diagnosed.The detection rate of neonatal hemolytic disease was 18.0%.Among 67 cases,the positive rate of red blood cell antibody release test was 100%.The positive rate of serum free antibody test was 62.7%,and the positive rate of direct anti-human globulin test was 1.4%.The detection rate of male children was 21.2%,and the detection rate of female children was 13.4%.There was no significant difference in comparison(P=0.052>0.05).The positive rate of the three tests of hemolysis is related to the time of specimen submission.The comparison of the two sets of data within two days and within 3 days and more than 3 days is statistically significant(P=0.003<0.05).The specific performance is:The test was submitted within 2 days,and the positive rate was high at 23.32%.Conclusion The three tests of hemolysis are of great significance in the diagnosis of hemolytic disease of the newborn,and the radiation test is the main basis for the diagnosis of hemolytic disease of the newborn.Children with clinical symptoms or suspected of hemolytic disease of the newborn should be tested as soon as possible.It's best to be tested within 2 days.
