经阴道尿道中段线性悬吊术治疗非复杂性压力性尿失禁的临床疗效观察

摘　　要：目的探究经阴道尿道中段线性悬吊术治疗非复杂性压力性尿失禁(SUI)的临床疗效。方法选取2016年12月至2019年1月在本院就诊的非复杂性SUI患者60例,采用随机数字表法分为观察组和对照组,每组各30例,对照组采用无张力尿道中段线性悬吊带术(TVT)治疗,观察组采用经阴道尿道中段线性悬吊术治疗,比较两组患者的临床疗效、术后生活质量及并发症发生情况。结果观察组的治疗总有效率显著高于对照组(P<0.05),手术时间、术中出血量、住院时间及术后并发症发生率显著低于对照组(P<0.05)。两组患者的术后尿失禁影响问卷简表(ⅡQ-7)评分均较术前下降(均P<0.001),且观察组高于对照组(P<0.001)。两组患者的术后盆腔器官脱垂-尿失禁性生活问卷简表(PISQ-12)评分均较术前升高(均P<0.05),且观察组高于对照组(P<0.05)。结论经阴道尿道中段线性悬吊术治疗非复杂性SUI临床疗效显著,可有效改善手术指标和患者的生活质量,减少术后并发症的发生。Objective To explore the clinical effect of transvaginal middle-urethral linear suspension in the treatment of uncomplicated stress urinary incontinence(SUI).Methods From December 2016 to January 2019,60 patients with uncomplicated SUI were selected and randomly divided into observation group and control group,30 cases in each group.The control group was treated with tension-free vaginal tape(TVT),and the observation group was treated with transvaginal middle-urethral linear suspension,and the clinical efficacy,postoperative quality of life and complications of the two methods in the treatment of uncomplicated SUI were evaluated.Results The total effective rate of the observation group was significantly higher than that of the control group(P<0.05),the time of operation,the amount of intraoperative bleeding during operation,the time of hospitalization and the incidence of postoperative complications were significantly lower than those in the control group(P<0.05).The incontinence impact questionnaire short form(ⅡQ-7)score of the two groups was lower than that before operation,and the observation group was significantly higher than the control group(P<0.05).The score of pelvic organprolapsed-urinary incontinence sexual questionnaire-12(PISQ-12)in the two groups was higher than that before operation,and the observation group was significantly higher than the control group(P<0.05).Conclusions The treatment of uncomplicated SUI by transvaginal middle-urethral linear suspension has a significant clinical effect,which can effectively improve the surgical indicators and the quality of life of patients,and reduce the incidence of postoperative complications.
