整肠生联合大蒜素胶囊预防腹膜透析患者真菌性腹膜炎的疗效研究

摘　　要：目的探讨整肠生联合大蒜素胶囊预防真菌性腹膜炎的疗效。方法回顾性分析本院2018年1月至2019年12月期间所有腹膜透析相关性腹膜炎(PDAP)患者的人口统计学资料、致病菌等资料,根据既往是否行整肠生联合大蒜素胶囊干预治疗分为对照组(60例)和干预组(101例)。共纳入符合标准的患者161例,根据病原微生物分为真菌组(14例)与非真菌组(147例)。结果干预组以及对照组在年龄、血肌酐、血清白蛋白、血红蛋白、心胸比等方面比较,差异均无统计学意义(均P>0.05);干预组真菌性腹膜炎的发生率明显低于对照组[4.0%(4/101)vs.16.7%(10/60),P<0.05]。真菌组和非真菌组在整肠生联合大蒜素胶囊干预以及血清白蛋白水平比较,差异有统计学意义(P=0.006、0.043)。多因素logistic回归分析表明整肠生联合大蒜素胶囊干预(OR=4.572,95%CI:1.346~15.528,P=0.015)是真菌性腹膜炎发生的独立影响因素。结论整肠生联合大蒜素胶囊预防腹膜透析(PD)患者真菌性腹膜炎的疗效显著。Objective To explore the efficacy of Zhengchangsheng combined with allicin capsules in preventing fungal peritonitis.Methods All patients who developed peritoneal dialysis-related peritonitis(PADP)between January 2018 and December 2019 in our hospital were enrolled,who were divided into control group(60 cases)and intervention group(101 cases)according to whether they had been treated with Zhengchangsheng combined with allicin capsule.Demographic data and pathogenic bacterial spectrum were analyzed.A total of 161 patients were enrolled,and were divided into fungal group(14 cases)and non-fungal group(147 cases)based on pathogenic diagnosis.Results There was no statistically significant difference between the intervention group and the control group in age,serum creatinine,serum albumin,hemoglobin,cardiothoracic ratio,etc(all P>0.05).The incidence of fungal peritonitis in intervention group was significantly lower than that in control group(4.0%vs.16.7%,P<0.05).There was statistically significant difference between the two groups in the intervention of Zhengchangsheng with allicin capsules and the serum albumin level(P=0.006,0.043).Multivariate logistic regression analysis showed that the intervention of Zhengchangsheng with allicin capsules(OR=4.572,95%CI:1.346-15.528,P=0.015)was an independent factor influencing the occurrence of fungal peritonitis.Conclusions Zhengchangsheng combined with allicin capsules is effective in preventing fungal peritonitis in peritoneal dialysis(PD)patients.
