摘  要:目的评价乙肝病毒(HBV)感染孕妇病毒载量对体液免疫状态及新生儿HBV感染率的影响。方法将2017年1月至2020年12月于河南医学高等专科学校附属医院产检并生产的200例HBV感染孕妇纳入本研究, 按照HBV DNA载量不同分为4组, 另选50例健康孕妇作为健康组。对比各组体液免疫功能[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、补体C3、补体C4]、肝功能指标[谷草转氨酶(ALT)、谷丙转氨酶(AST)、谷氨酰转肽酶(GGT)]及新生儿HBV感染率、围产期并发症发生率。结果高载量组血清IgG、IgM水平>中载量组>低载量组>阴性组>健康组, 高载量组补体C3、补体C4水平<中载量组<低载量组<阴性组<健康组, 差异有统计学意义(P<0.05);高载量组血清ALT、AST、GGT水平>中载量组>低载量组>阴性组>健康组, 差异有统计学意义(P<0.05);高载量组新生儿HBV感染率、并发症发生率均比中载量组、低载量组、阴性组、健康组高, 差异有统计学意义(P<0.05)。结论 HBV感染孕妇HBV DNA载量上升, 会增加孕妇体液免疫紊乱、肝功能受损及新生儿HBV感染、并发症发生风险, 故临床可动态监测HBV DNA载量, 以指导临床治疗。Objective To evaluate the effects of hepatitis B virus(HBV)viral load of HBV-infected pregnant women on the humoral immune status and neonatal HBV infection rate.Methods Two hundred cases of HBV-infected pregnant women who had undergone an obstetric examination and delivered in the Affiliated Hospital of Henan Medical College from January 2017 to December 2020 were enrolled in this study.The patients were divided into 4 groups according to different HBV DNA loads.Another 50 healthy pregnant women were selected as healthy group.The humoral immune function[immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM),complement C3,complement C4],liver function indicators,[aspartate aminotransferase(ALT),Alanine aminotransferase(AST)and glutamyl transpeptidase(GGT)],neonatal HBV infection rate and perinatal complications of each group were compared.Results Serum IgG and IgM levels were the highest in high-load,medium-load,low-load,followed by high-load,medium-load,low-load,negative group and healthy group.The levels of complement C3 and complement C4 were the lowest in high-load,medium-load,low-load and successively increased in high-load,medium-load,low-load,negative group and healthy group.The differences were statistically significant(P<0.05).The neonatal HBV infection rate and complication rate of the high-load group were higher than those of the medium-load group,low-load group,negative group and healthy group,and the differences were statistically significant(P<0.05).Conclusions The increment of HBV DNA load of HBV-infected pregnant women will increase the risk of humoral immune disorder,liver function damage as well as HBV infection and complications in newborns.
