摘 要:目的探讨侵袭性感染婴儿的B群链球菌(GBS)菌株血清学分布特点、耐药性及相关耐药基因的携带情况。方法收集2017至2021年江西省儿童医院患儿无菌部位分离获得的GBS菌株。采用Phoneix-100全自动细菌分析系统对菌株进行鉴定及抗菌药物最低抑菌浓度检测(MIC);D试验检测菌株对红霉素的耐药表型;普通PCR方法检测菌株的荚膜血清型及对红霉素、克林霉素和四环素的耐药基因。结果62株B群链球菌共检出4种荚膜血清型(Ⅰa、Ⅰb、Ⅲ、Ⅴ),以Ⅲ型最常见(51.6%),其次为Ⅰb型(33.6%)、Ⅰa型(12.9%)及Ⅴ型(1.6%)。GBS对红霉素、克林霉素、四环素和左氧氟沙星耐药率分别为83.9%、80.6%、69.4%和17.7%。红霉素耐药菌株ermB检出率最高(86.5%),Ⅰb型均为cMLSB耐药表型,均携带ermB、tetO基因。结论本地区婴儿GBS侵袭性感染以Ⅲ和Ⅰb型为主,不同血清型GBS菌株对红霉素、克林霉素和四环素耐药性差别有统计学意义,Ⅰb和Ⅲ型较Ⅰa型表现出更高的耐药性。部分血清型与耐药基因呈明显相关性。Objective To analyze serotypes distribution and related resistance genes of Group strepotoccus(GBS)causing invasive infections in infants.Methods Invasive strains of GBS were collected from infants in Jiangxi Provincial Children’s Hospital during 2017 to 2021.The GBS isolates were identified and the minimum inhibitory concentration(MIC)of the antibiotics was determined by Phoneix-100 automatic bacterial identification system.GBS serological typing,the presence of resistance genes to erythromycin,Clindamycin and tetracycline were determined with PCR amplification.Results Four serotypes were found from 62 GBS strains.SerotypeⅢ(51.6%)was the most common,followed byⅠb(33.6%),Ⅰa(12.9%)andⅤ(1.6%).The resistance of the isolates to erythromycin,clindamycin,tetracyline and levofloxacin were 83.9%,80.6%,69.4%,17.7%.The most frequently identified resistance gene was ermB(86.5%).All ofⅠb strains were cMLSB phenotypes and carrying ermB and tetO genes.Conclusion The resistance ofⅢandⅠb were higher thanⅠa serotypes,there were significant differences to erythromycin,clindamycin and tetracyline between them.Some serotypes were significantly correlated with antimicrobial resistance genes.
