摘  要:目的 探究妊娠期甲状腺功能减退患者肠道菌群特征,为该类患者的治疗提供参考。方法 选择2018年12月至2019年12月于大连大学附属中山医院住院分娩的妊娠期甲状腺功能减退患者8例为甲减组,选择同期健康妊娠期妇女14例为对照组。收集两组对象孕晚期粪便样本,采用16S rRNA测序法对其肠道菌群进行分析。结果 两组对象肠道菌群构成存在明显差异,甲减组患者肠道菌群Simpson指数显著降低(t=-2.093,P=0.023),Ace指数(t=-2.025,P=0.003)及Chao1指数(t=-1.922,P=0.022)较对照组显著升高。与对照组相比,甲减组患者肠道厚壁菌门的丰度增高,变形菌门和放线菌门的丰度降低;在属水平上,甲减组患者肠道Dialister、Blautia、unidentified;uminococcaceae、unidentified;nterobacteriaceae和Subdoligranulum丰度增加,Bifidobacterium、Phascolarctobacterium和Streptococcus丰度降低。结论 妊娠期甲状腺功能减退患者肠道菌群存在明显失调。Objective To explore the characteristics of intestinal microbiota in patients with hypothyroidism during pregnancy. Methods Eight patients with gestational hypothyroidism who were hospitalized for delivery in our hospital from December 2018 to December 2019 were enrolled as hypothyroidism group and 14 healthy pregnant women during the same period as the control group. Stool samples of the pregnant women in the third trimester of pregnancy were collected from the two groups, and stool microbiota was analyzed with 16 S rRNA sequencing. Results There were significant differences in the composition of intestinal microbiota between the two groups. The Simpson value of intestinal microbiota significantly decreased in hypothyroidism group(t=-2.093, P=0.023);Ace index(t=-2.025, P=0.003) and Chao1 index(t=-1.922, P=0.022) were significantly higher than in the control group. Compared with the control group, the abundance of Firmicutes increased and those of Proteobacteria and Actinomycetes decreased in hypothyroidism group. At the genus level, the abundances of Dialister, Blautia, unidentified;uminococcaceae, unidentified;nterobacteriaceae and Subdoligranulum increased, while those of Bifidobacterium, Phascolarctobacterium and Streptococcus decreased in hypothyroidism group. Conclusion The intestinal microbiota of pregnant patients with hypothyroidism is obviously disordered.
