回肠结核并发右侧腹股沟寒性脓肿1例影像特点并文献分析

摘　　要：目的 探讨腹股沟区寒性脓肿的影像学表现并结合文献进行分析,为日后诊断肠结核脓肿提供有利的支持。方法 回顾分析重庆市南川区中医院1例肠结核并形成脓肿至右侧腹股沟区的临床资料、影像学表现并文献复习。结果 腹股沟区寒性脓肿特征性表现为向周围蔓延且呈流注样下行,无向上蔓延趋势,CT增强扫描脓肿环形强化,MRI表现多呈T;WI低T;WI高或略高的混杂信号。结论 肠结核并发腹股沟区寒性脓肿十分罕见,可通过CT、MRI等影像检查进行鉴别,提高术前诊断率,为临床医生制定治疗方案提供有效依据,减少误诊、漏诊机会。Objective To explore the imaging manifestations of cold abscess in the groin area and analyze it in combination with the literature to provide favorable support for the diagnosis of intestinal tuberculosis abscess in the future.Method A case of Intestinal tuberculosis with abscess to the right groin area was analyzed retrospectively.Result The characteristic manifestation of cold abscess in the groin area is spreading to the periphery and descending in a stream-like manner,with no upward spreading trend.The abscess in enhanced CT scans is enhanced in a ring shape,MRI manifestations are mostly confounding signals with low T;WI and high or slightly higher T;WI.Conclusion Intestinal tuberculosis complicated by cold abscess in the inguinal area is very rare.It can be differentiated by CT,MRI and other imaging findings to improve the preoperative diagnosis rate,provide an effective basis for clinicians to formulate treatment plans,and reduce the chance of misdiagnosis and missed diagnosis.
