柴胡栀子汤联合甲硫咪唑对甲状腺功能亢进患者甲状腺激素作用探讨

摘　　要：目的观察柴胡栀子汤联合甲硫咪唑对甲状腺功能亢进(HT)患者甲状腺激素的作用。方法将我院134例HT患者随机分为对照组与观察组各67例。观察组采用口服柴胡栀子汤+甲硫咪唑治疗,对照组采用口服甲硫咪唑治疗,比较两组患者临床疗效,治疗前后患者中医症候得分、甲状腺激素、不良反应和复发情况。结果治疗后,观察组总有效率显著高于对照组(P<0.05);两组患者胸胁胀闷、大便溏薄、怕热多汗、心悸失眠评分均降低,且观察组患者胸胁胀闷、大便溏薄、怕热多汗、心悸失眠评分均低于对照组(P<0.05);两组患者游离甲状腺素T3(FT3)、游离甲状腺素T4(FT4)、总三碘甲状腺原氨酸(TT3)、总甲状腺素(TT4)均下降,促甲状腺激素(TSH)升高,且观察组患者FT3、FT4、TT3、TT4均低于对照组,TSH高于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05),观察组复发率低于对照组(P<0.05)。结论柴胡栀子汤联合甲硫咪唑治疗HT疗效确切,能缓解患者临床症状,调节甲状腺激素水平,降低复发率,且不增加不良反应。Objective To observe the effect of Chaihu Zhizi Decoction combined with methimazole on thyroid hormones in patients with hyperthyroidism(HT).Methods A total of 134 HT patients were randomly divided into control group and observation group with 67 cases in each.Observation group was treated with oral Chaihu Zhizi Decoction plus methimazole,and control group was treated with oral methimazole.The clinical efficacies and the scores of TCM symptoms,thyroid hormones,adverse reactions and recurrences of the two groups before and after treatment were compared.Results After treatment,the total effective rate of observation group was significantly higher than control group(P<0.05);the scores of both groups with chest and thoracic swelling,loose stools,heat resistance and sweating,palpitation and insomnia were decreased,and the scores of observation group were lower(P<0.05);free thyroxine T3(FT3),free thyroxine T4(FT4),total triiodothyroxine(TT3)and total thyroxine(TT4)decreased,and thyroid-stimulating hormone(TSH)increased,and FT3,FT4,TT3,TT4 of observation group were lower than control group,and TSH was higher than control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05),and the recurrence rate of observation group was lower than control group(P<0.05).Conclusion Chaihu Zhizi Decoction combined with methimazole has a definite effect in treatment of HT.It can relieve the clinical symptoms of patients,regulate the level of thyroid hormones,reduce the recurrence rate,and won’t increase adverse reactions.
