柏氏清热散瘀方对肛裂术后创面血管生成及VEGF水平的影响

摘　　要：目的观察柏氏清热散瘀方对肛裂术后创面血管生成及血管内皮生长因子(VEGF)水平的影响。方法将我院行括约肌切开术治疗的肛裂104例患者随机分为观察组与对照组各52例,对照组给予高锰酸钾液熏洗治疗,观察组给予柏氏清热散瘀方熏洗治疗,比较两组临床疗效、视觉模拟(VAS)评分、创面愈合率、毛细血管数量、VEGF水平、创面愈合时间和出血消失时间。结果观察组的有效率显著高于对照组(P<0.05);术后7d、术后14d观察组的VAS评分均显著低于同期对照组(P<0.05);观察组术后7d和术后14d患者创面愈合率、毛细血管数量和VEGF水平均显著高于对照组(P<0.05);观察组患者创面愈合时间和出血消失时间均显著低于对照组(P<0.05)。结论柏氏清热散瘀方能显著减轻肛裂术后患者创面疼痛,促进创面新血管生成,提高创面组织VEGF表达水平,加速创面愈合。Objective To observe the effect of Bai’s Qing’re Sanyu Formula on angiogenesis and vascular endothelial growth factor(VEGF)level of wound after anal fissure.Methods Patients with anal fissure treated by sphincterotomy were randomly divided into observation group and control group with 52 cases in each group.Control group was treated with potassium permanganate solution fumigation and washing,and observation group was treated with Bai’s Qing’re Sanyu Formula.The clinical efficacy,Visual analogue scale(VAS)score,wound healing rate,capillary number,VEGF level,wound healing time and hematochezia extinction time were compared between the two groups.Results The clinical efficacy of the observation group was significantly better than that of control group(P<0.05);the VAS score of observation group was significantly lower than that of the control group 7 d and 14 d after operation(P<0.05);the wound healing rate,capillary number and VEGF level in observation group were significantly higher than those in control group(P<0.05);both wound healing time and hematochezia extinction time in observation group were significantly lower than control group(P<0.05).Conclusion Bai’s Qing’re Sanyu Formula can significantly reduce wound pain after anal fissure surgery,promote new wound angiogenesis,improve the expression level VEGF wound tissue and accelerate wound healing.
