推拿桥弓结合针刺治疗桥本氏甲状腺炎临床研究

摘　　要：目的观察推拿桥弓结合针刺治疗桥本氏甲状腺炎的临床疗效。方法将66例桥本氏甲状腺炎患者随机分为观察组与对照组,共脱失4例,观察组完成30例,对照组完成32例。观察组予以推拿桥弓结合针刺治疗,对照组给予优甲乐治疗。观察治疗前后两组患者的甲状腺功能水平、甲状腺特异性抗体滴度、甲状腺体积和峡部厚度等变化。结果治疗后,两组患者甲状腺激素均较治疗前有明显变化,组间治疗后比较,观察组促甲状腺激素(TSH)降低幅度大于对照组(P<0.05),游离T3(FT3)、游离T4(FT4)升高幅度大于对照组(P<0.05);抗甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)与甲状腺微粒体抗体(TMAb)滴度均较治疗前降低(P<0.05);观察组甲状腺体积和峡部厚度炎性肿胀均较治疗前降低(P<0.05),而对照组甲状腺体积和峡部厚度炎性肿胀为部分降低;观察组综合疗效优于对照组(P<0.05)。结论推拿桥弓结合针刺对改善桥本氏甲状腺炎患者的临床症状、甲状腺功能有显著效果,可作为治疗桥本氏甲状腺炎的可行性方案,在临床上推广应用。Objective To observe the clinical effect of Qiaogong Point Tuina combined with acupuncture on Hashimoto’s thyroiditis(HT).Methods A total of 66 cases of patients with HT were randomly divided into treatment group and the control group,4 cases lost,30 cases in treatment group,32 cases in control group.Treatment group was treated with Qiaogong Point Tuina combined with acupuncture,and control group was treated with Euthyrox.Thyroid function,thyroid specific antibody titer,thyroid volume and isthmus thickness were observed before and after treatment.Results After treatment,the thyroid hormone of both groups had significant changes compared with before treatment;the decrease of thyroid stimulating hormone(TSH)in treatment group was greater than control group(P<0.05),and the increase of free T3(FT3)and free T4(FT4)in treatment group was greater than control group(P<0.05);the titers of TPOAb,TGAb and TMAb were lower than those before treatment(P<0.05);the thyroid volume and isthmus thickness of treatment group were lower than before treatment(P<0.05),while the thyroid volume and isthmus thickness of control group were lower than before treatment(P<0.05);the comprehensive effect of treatment group was better than control group(P<0.05).Conclusion Qiaogong Point Tuina combined with acupuncture has significant effect on improving the clinical symptoms and thyroid function of patients with HT,which can be used as a feasible scheme for the treatment of HT.
