肾移植术后PRA抗体对受者预后影响的相关性研究

摘　　要：目的:探讨肾移植术后PRA抗体与移植肾的肾功能、病理表现及受者预后的关系。方法:选择2014-01~2016-06行同种异体肾移植术的患者86例,根据PRA检测结果将患者分成PRA阳性组和PRA阴性组。观察两组移植肾的存活率、肾功能及病理表现。结果:PRA阴性组术后移植物存活率明显高于PRA阳性组(P<0.01);PRA阴性组的移植肾功能明显优于PRA阳性组(P<0.05);PRA阳性组的病理结果中,出现血管内膜炎、间质炎症细胞浸润、小管炎、管周毛细血管炎、C4d阳性的病例数明显比PRA阴性组高(P<0.01)。结论:肾移植术后PRA抗体阳性能引起排斥反应,损害受者的肾功能,降低移植肾的长期存活率;肾移植术前和术后通过检测PRA水平,可更好的预防和治疗排斥反应,提高移植肾的长期存活率。Objective:To investigate the relationship between PRA antibody and renal function,pathological features and prognosis of transplanted kidney after renal transplantation.Methods:A total of 86 patients who underwent renal allotransplantation from January 2014 to June 2016 were selected and divided into PRA positive group and PRA negative group according to the results of PRA test.The survival rate,renal function and pathological features of transplanted kidneys were observed.Results:The graft survival rate of PRA negative group was significantly higher than PRA positive group(P<0.01).The transplanted renal function in PRA negative group was significantly better than that in PRA positive group(P<0.05).In the PRA positive group,the number of patients with endovascular inflammation,interstitial inflammatory cell infiltration,microangiitis,peritubular capillaries,and POSITIVE C4d was significantly higher than that in the PRA negative group(P<0.01).Conclusion:Positive PRA antibody after renal transplantation can lead to rejection,damage the renal function of the recipient,and reduce the long-term survival rate of the transplanted kidney.Detection of PRA levels before and after renal transplantation can better prevent and treat rejection and improve long-term survival of transplanted kidneys.
