相似形改良旋转皮瓣的临床应用体会

摘　　要：目的探讨相似形改良旋转皮瓣修复外伤创伤、褥疮、肿物切除术等原因导致的皮肤缺损的效果。方法自2014年3月至2019年5月,哈尔滨医科大学附属第四医院整形美容科收治因头面部外伤、褥疮、色素痣、肿物切除后导致皮肤缺损的27例(缺损27处)患者,采用相似形改良旋转皮瓣修复,术中彻底切除病变组织后,设计相似形改良旋转皮瓣,在适当层次充分游离组织形成皮瓣后修复创面。结果所有患者术后均无血运障碍,皮瓣成活,切口均一期愈合,局部未见明显变形,患者对修复效果满意。结论采用相似形改良旋转皮瓣修复皮肤缺损具有皮瓣成活率较高,不破坏局部原有生理弧度,瘢痕长度较短,局部张力较小,瘢痕不明显,修复效果良好的特点。Objective To explore the clinical efficacy of modified rotation skin flap with similar shape in the repair of skin defects caused by trauma,bedsore and tumor resection.Methods From March 2014 to May 2019,27 patients with skin defects caused by trauma of the head and face,bedsore,pigmented nevus and tumor resection were repaired by modified rotation skin flap with similar shape.After the pathological tissue was completely excised during the operation,the modified rotation skin flap with similar shape was designed to repair the wounds at suitable layer after sufficient free tissue was formed.Results No blood circulation disorder was found in all patients after operation.The flaps survived and the incisions healed by first intention.No obvious local deformation was found and patients were satisfied with the efficacy.Conclusion The modified rotation skin flap with similar shape has the advantages of high survival rate of skin flap,complete local physiological curvature,short length of scar,low local tension,concealed scar and good efficacy of repair.
