掌背星状皮瓣指/趾蹼成形在并指/趾畸形中的临床应用

摘　　要：目的探讨应用掌背星状皮瓣指/趾蹼成形矫正并指/趾畸形的临床疗效。方法自2018年3月至2020年5月,海军第九七一医院手外科应用星状皮瓣指/趾蹼成形治疗并指/趾畸形患者17例,其中先天性手指并指畸形7例,外伤皮瓣修复术后并指畸形4例,足并趾畸形6例。术中应用掌背星状皮瓣重建指/趾蹼,指/趾体间采用连续Z形皮瓣分离术,记录患者术中分指/趾是否植皮,术后有无切口感染、皮瓣坏死、指蹼形态、瘢痕情况及远期是否有指蹼前移。结果本组所有患者成形指/趾蹼均未植皮,创面直接闭合,术后伤口均一期愈合,未发生皮瓣坏死、伤口感染、不愈合等并发症;术后随访3~12个月,指蹼形态宽度、深度、坡度正常,瘢痕隐蔽,分离指/趾体未见明显瘢痕挛缩。结论采用掌背星状皮瓣指/趾蹼成形矫正并指/趾畸形,手术安全,术中无需植皮,手术时间较短,术后指蹼形态较好,瘢痕隐蔽,是一种较为满意的方法。Objective To explore the clinical efficacy of dorsal metacarpal star flap in the reconstruction of finger/toe web in syndactyly.Methods From March 2018 to May 2020,17 patients with syndactyly were selected.There were 7 cases of congenital syndactyly,4 cases of syndactyly caused by flap repair of trauma and 6 cases of toe syndactyly.The dorsal metacarpal star flap was used in the reconstruction of finger/toe web and the continuous Z-shaped flap separation was used among the fingers/toes.The condition of skin graft,incision infection,skin flap necrosis,morphology of finger/toe web,scar and long-term finger/toe web advancement were recorded.Results No skin graft was performed in all patients.The wounds closed directly and healed by first intention.No complications such as flap necrosis,incision infection and nonhealing occurred.All patients were followed-up for 3 to 12 months,the morphology of finger/toe web was normal and no obvious scar contracture was found.Conclusion Application of dorsal metacarpal star flap in the reconstruction of finger/toe web was safe.There was no need of skin graft during the operation.The operative time was short,the morphology of finger/toe web after operation was good and the scar was concealed.This method was satisfactory.
