摘　　要：临床药师参与1例罕见的急性起病并继发噬血细胞综合征的黑热病患者的药学实践,通过多学科急会诊、临床药学小组紧急会诊、重点病患全程药学监护以及诊疗效果的追踪分析,探讨了急诊临床药师的服务模式。该模式可充分发挥急诊药师作用,为急诊患者提供快速、有效、安全、个体化的药学服务,同时为临床治疗积累了经验:对于急性起病、高龄或伴合并症的黑热病患者,初始治疗方案宜选用两性霉素B脂质体。通过践行急诊药学服务模式,对临床药师深入开展急诊药学服务具有一定意义。Clinical pharmacist participated in the pharmaceutical practice for a rare kala-azar patient with acute onset and secondary hemophagocytic syndrome.The service model of emergency clinical pharmacists was discussed through multi-disciplinary emergency consultation,clinical pharmacy group emergency consultation,whole-process pharmaceutical care for key patients and follow-up analy・sis of treatment effects.This model can give full play to the role of emergency pharmacists,provide rapid,effective,safe and personalized pharmaceutical care for emergency patients,and accumulate experience for clinical treatment of kala-azar patients with acute onset,advanced age or complications,and amphotericin B liposome may be more conducive to timely control of the disease in the initial treatment with higher safety.Through practicing the mode of emergency pharmaceutical care,it has certain significance for clinical pharmacists to carry out in-depth emergency pharmaceutical care.
