摘 要:目的研究手卫生集束化管理对新生儿重症监护室(NICU)工作人员手卫生依从性和正确率的影响,以及在医院感染控制中的作用。方法收集2015年1月—2019年12月NICU工作人员手卫生执行情况,将采取常规管理的2015—2016年手卫生情况作为对照组,采取集束化管理的2017—2019年手卫生情况作为观察组,比较两组NICU工作人员手卫生依从率、正确率及医院感染发生率。结果NICU手卫生不同时机依从性对照组为60.57%,观察组为84.17%;不同工种依从性对照组为61.70%,观察组为79.11%,两组依从性差异均有统计学意义(P<0.05)。手卫生不同时机正确率对照组为67.65%,观察组为85.09%;不同工种正确率对照组为68.20%,观察组为85.01%,两组正确率差异均有统计学意义(P<0.05)。NICU医院感染发生率对照组为3.11%,观察组为2.49%,两组差异无统计学意义(P>0.05);日感染率对照组为3.68‰,观察组为2.43‰,两组差异有统计学意义(P<0.05)。结论手卫生集束化管理能有效提高工作人员手卫生依从性及正确率,降低院内感染风险。Objective To study the effect of management bundles for hand hygiene on the compliance and accuracy of hand hygiene of neonatal intensive care unit(NICU)staff and its role in nosocomial infection control.Methods The implementation of hand hygiene of NICU staff from January 2015 to December 2019 was collected.The routine management of hand hygiene from 2015 to 2016 was set as the control group,and the management bundles for hand hygiene from 2017 to 2019 was used as the observation group.The compliance rate and accuracy of hand hygiene and the incidence of nosocomial infection of NICU staff were compared between the two groups.Results The compliance of NICU hand hygiene at different times was 60.57%in the control group and 84.17%in the observation group;the compliance of different types of work was 61.70%in the control group and 79.11%in the observation group,and the differences in compliance between the two groups were statistically significant(P<0.05).The accuracy of hand hygiene at different time was 67.65%in the control group and 85.09%in observation group;the accuracy of different types of work was 68.20%in the control group and 85.01%in the observation group,and the differences in accuracy between the two groups were statistically significant(P<0.05).The incidence of NICU nosocomial infection was 3.11%in the control group and 2.49%in the observation group,and there was no significant difference between the two groups(P>0.05).The daily infection rate was 3.68‰in the control group and 2.43‰in the observation group,and the difference between the two groups was statistically significant(P<0.05).Conclusion Management bundles for hand hygiene can effectively improve the hand hygiene compliance and accuracy of staff,and reduce the risk of nosocomial infection.
