摘 要:目的研究绍兴市部分医院血流感染肺炎克雷伯菌的耐药性,为临床合理使用抗菌药物提供依据。方法通过细菌分离鉴定技术和药敏试验,对绍兴地区不同医院住院患者送检的血液标本进行肺炎克雷伯菌分离培养和药敏试验观察。结果4家医院共查出136例血流感染肺炎克雷伯菌患者,其中男性89例,女性47例,老年患者占82.35%。所分离的肺炎克雷伯菌对阿米卡星和替加环素耐药率较低,对亚胺培南和厄他培南耐药率分别为22.79%和24.26%。在136株血流感染肺炎克雷伯菌中共检出31株产超广谱β-内酰胺酶阳性菌株、33株耐碳青霉烯类肺炎克雷伯菌株。结论血流感染肺炎克雷伯菌对临床常用抗菌药物普遍耐药,耐碳青霉烯类菌株比例较高。Objective To study the drug resistance of Klebsiella pneumoniae causing bloodstream infection in different hospitals in Shaoxing,and to provide a basis for the clinical rational use of antibiotics.Methods Through bacterial isolation and identification technology and drug susceptibility test,Klebsiella pneumoniae were isolated and cultured from blood samples submitted by inpatients from different hospitals in Shaoxing district,and were observed for drug susceptibility tests.Results A total of 136 patients with bloodstream infection caused by Klebsiella pneumonia were detected in 4 hospitals,of which 89 were males and 47 were females,and elderly patients accounted for 82.35%.The isolated Klebsiella pneumoniae had low resistance rates to amikacin and tigecycline,and the resistance rates to imipenem and ertapenem were 22.79%and 24.63%,respectively.A total of 31 ESBL-positive strains and 33 carbapenem-resistant strains were detected among 136 strains of Klebsiella pneumoniae causing bloodstream infection.Conclusion The Klebsiella pneumoniae causing bloodstream infection are generally resistant to commonly used clinical antibacterial drugs,and the proportion of carbapenem-resistant strains is relatively high.
