摘 要:目的分析饮酒与血尿酸水平的关联,为降低中国居民血尿酸水平提供依据。方法从2010—2011年全国15家三级甲等医院内分泌科就诊患者中连续入组糖代谢正常(843人)、糖调节受损(499人)、新诊断2型糖尿病(509人)及已诊断2型糖尿病患者(1214人)4组人群共3065例。采集人口学资料、现病史、既往史、吸烟和饮酒信息,并进行体格检查、血尿酸水平测定。采用R 3.4.3软件进行t检验、χ^(2)检验、Wilcoxon秩和检验、多重线性回归分析和交互作用分析。结果多重线性回归模型结果显示,与经常不饮酒者相比,目前经常饮酒者血尿酸高41.89μmol/L(95%CI:27.48~56.29μmol/L),差异有统计学意义(P<0.01)。目前饮酒和既往饮酒已戒酒人群血尿酸水平均显著高于不饮酒人群(分别高20.52和22.93μmol/L),差异有统计学意义(P<0.05,P<0.01)。在戒酒人群中,戒酒<5年者血尿酸高于戒酒≥5年者(高53.42μmol/L),差异有统计学意义(P<0.01)。随着饮酒种类增加,血尿酸呈上升趋势(P<0.01)。随着酒精摄入量的升高,血尿酸水平呈上升趋势(P<0.05)。亚组分析发现,吸烟情况和糖代谢类型可能影响饮酒与血尿酸的关联,交互作用检验有统计学意义(t值分别为2.57和3.19,P<0.01)。结论饮酒与血尿酸升高存在明显关联,不同吸烟情况和糖代谢状态的人群中饮酒与血尿酸的关联存在差异。Objective To analyze the relationship between drinking and uric acid level,and provide the basis for reducing the uric acid level of Chinese residents.Methods From 2010 to 2011,843 cases with normal glucose metabolism,499 case with impaired glucose regulation,509 cases with type 2 diabetes mellitus newly diagnosed and 1214 cases with diagnosed type 2 diabetes mellitus(total 3065 cases)from endocrinology departments of 15 hospitals with GradeⅢA in whole China served as the subjects.The investigation was performed with collecting information(including the demography data,current medical history,past medical history,smoking and drinking),physical examination and uric acid test.The t test,χ^(2) test,Wilcoxon rank sum test and the multiple linear regression interactive analysis were used to analyze the data.Results The multiple linear regression showed that as compared to subjects with infrequent drinking,the uric acid level of subjects with regular drinking significantly increased(41.89μmol/L,95%CI:27.48-56.29μmol/L),P<0.01;the uric acid level in subjects with current drinking or subjects with quitting alcohol was significantly higher(20.52μmol/L and 22.93μmol/L,respectively)than that in subjects without drinking(P<0.05).In subjects quitting alcohol,the uric acid level of subjects quitting alcohol for<5 years was significantly higher(53.42μmol/L)than that of subjects quitting alcohol for≥5 years(P<0.01).The uric acid level trend increased with alcohol type(P<0.01).The uric acid level trend increased with alcohol consumption(P<0.05).The analysis of sub-groups showed that smoking and glucose metabolism type could influence the correlation between drinking and blood uric acid level,there was statistics significance of interaction test(t values were 2.57 and 3.19,P<0.01).Conclusion There is significant correlation between drinking and enhanced blood uric acid level,in the subjects with different smoking and glucose metabolism,there is significant difference of correlation between drinking and blood uric acid.
