摘　　要：目的了解艾滋病儿童病例抗病毒治疗影响因素,提高治疗效果。方法采用回顾性队列研究的方法,选取南阳市2003-2020年间接受抗病毒治疗的艾滋病儿童病例作为研究对象,从抗病毒治疗效果、影响因素等方面分析。结果144例艾滋病儿童病例从CD4计数均值变化和病毒抑制率分析,抗病毒治疗效果显著(P<0.01),影响因素中有漏服现象的病例和开始抗病毒治疗时临床分期Ⅲ期的病例发生死亡或退出治疗的可能性高(OR=8.75,P<0.01;OR=6.51,P<0.05),而确证后1年内抗病毒治疗病例发生死亡或退出治疗的可能性低(OR=0.30,P<0.01)。结论艾滋病儿童确证后及时治疗,减少抗病毒治疗漏服情况,可以提高艾滋病儿童抗病毒治疗效果和生存时间。Objective To understand the influencing factors of antiviral therapy for AIDS children and improve the therapeutic effect.Methods Using the method of retrospective cohort study,the AIDS cases of children who received antiviral treatment in Nanyang city from 2003 to 2020 were selected as the research object,and the effect of antiviral treatment and influencing factors were analyzed.Results The antiviral treatment effect was significant among the 144 cases of AIDS children after analysis of the average change of CD4 count and the rate of virus inhibition(P<0.01).Among the influencing factors,there was a high possibility of death or withdrawal from treatment in cases with missed medication and cases in clinical stage III when antiviral treatment was started(OR=8.75,P<0.019;OR=6.51,P<0.05).However,the probability of death or withdrawal from antiviral therapy within one year after confirmation was low(OR=0.30,P<0.01).Conclusion Prompt treatment for AIDS children after confirmation and reducing missed antiretroviral treatment can improve the efficacy of antiretroviral treatment and survival time for AIDS children.
