金栀洁龈含漱液联合米诺环素治疗慢性牙周炎的临床研究

摘　　要：目的探讨金栀洁龈含漱液联合米诺环素治疗慢性牙周炎的临床疗效。方法选取2020年3月—2021年8月在新乡医学院第三附属医院收治的122例慢性牙周炎患者,共有患牙183颗,根据随机数字表法将所有患者分为对照组和治疗组,每组各有61例。对照组将盐酸米诺环素软膏注入牙周袋内,略有溢出,1次/周。治疗组在对照组基础上给予金栀洁龈含漱液,每次使用10 mL含漱15 min,6次/d。两组患者连续治疗4周。观察两组临床疗效,比较两组治疗前后的牙周指标、生活质量和龈沟液中前列腺素E2(PGE2)、白细胞介素-8(IL-8)水平。结果治疗后,治疗组总有效率(86.69%)明显比对照组(70.49%)高,差异有统计学意义(P<0.05)。治疗后,两组的探诊深度、牙龈指数、菌斑指数显著降低(P<0.05),且治疗后治疗组的探诊深度、牙龈指数、菌斑指数比对照组更低(P<0.05)。治疗后,两组的口腔健康影响程度量表(OHIP-49)评分显著降低(P<0.05),且治疗组患者OHIP-49评分明显低于对照组(P<0.05)。治疗后,两组龈沟液PGE2、IL-8水平显著低于治疗前(P<0.05);且治疗后治疗组龈沟液PGE2、IL-8低于对照组,差异有统计学意义(P<0.05)。结论金栀洁龈含漱液联合盐酸米诺环素软膏治疗慢性牙周炎的疗效确切,有助于减轻牙周症状,改善患者生活质量,降低炎症反应,药物安全性良好。Objective To investigate the effect of Jinzhi Jieying Gargle combined with Minocycline Hydrochloride Ointment in treatment of chronic periodontitis.Methods Patients(122 cases,183 teeth)with chronic periodontitis in the Third Affiliated Hospital of Xinxiang Medical University from March 2020 to August 2021 were randomly divided into control and treatment groups,and each group had 61 cases.Patients in the control group were injected into the periodontal bag with Minocycline Hydrochloride Ointment,and slight overflow,once weekly.Patients in the treatment group were given Jinzhi Jieying Gargle on the basis of the control group,rinsed 10 m L each time for 15 min,six times daily.Patients in two groups were treated for 4 weeks.After treatment,the clinical efficacies were evaluated,and periodontal indexes,quality of life,and the levels of PGE2 and IL-8 in gingival crevicular fluid of two groups were compared.Results After treatment,the total effective rate of the treatment group(86.69%)was significantly higher than that of the control group(70.49%)(P<0.05).After treatment,the probing depth,gingival index,and plaque index of two groups were significantly decreased(P<0.05),and the probing depth,gingival index,and plaque index of the treatment group were lower than those of the control group(P<0.05).After treatment,the OHIP-49 score of two groups was significantly decreased(P<0.05),and the OHIP-49 score of the treatment group was significantly lower than that of the control group(P<0.05).After treatment,the levels of PGE2 and IL-8 in gingival crevicular fluid of two groups were significantly lower than those before treatment(P<0.05).After treatment,the levels of PGE2 and IL-8 in gingival crevicular fluid of the treatment group were lower than those in the control group(P<0.05).Conclusion Jinzhi Jieying Gargle combined with minocycline is effective in treatment of chronic periodontitis,which can reduce periodontal symptoms,improve patients’quality of life,and reduce inflammatory reaction,with good drug safety.
