摘 要:目的探讨维生素D联合寒喘祖帕颗粒治疗支气管哮喘(BA)伴肺炎支原体肺炎(MPP)患儿血清白介素12(IL-12)、髓过氧化物酶(MPO)水平的影响。方法选取2019年3月—2021年3月在郑州大学附属儿童医院确诊的82例BA伴MPP患儿,采用简单随机分组将患儿分为观察组和对照组,每组各41例。对照组患儿采用维生素D治疗,观察组采用维生素D联合寒喘祖帕颗粒治疗,均持续治疗4周。分别于治疗前后,采用酶联免疫法测定血清白介素12(IL-12)、血清IgE水平及C反应蛋白(CRP)。采用流式细胞仪测定法检测中性粒细胞浆中MPO含量。采用肺功能检测仪测定一秒末用力呼气量(FEV1)、肺活量(PVC)水以及计算FEV1/FVC比值。记录两组治疗期间的不良反应发生情况。结果观察组患儿治疗的有效率为90.24%,明显高于对照组患儿治疗的73.17%(P<0.05);治疗后,观察组IL-12水平和FVC、FEV1和FEV1/FVC指标分别为(227.53±81.56)pg/L、(3.29±0.26)L、(2.78±0.42)L、(76.54±5.93)%,对照组分别(186.72±86.92)pg/L、(2.93±0.31)L、(2.26±0.52)L、(72.88±4.53)%,观察组明显高于对照组(P<0.05);观察组血清IgE和MPO水平分别(8.57±1.59)U/mL、(653.27±46.92)U/L,对照组分别(10.62±2.10)U/mL、(702.65±56.21)U/L,观察组明显低于对照组(P<0.05)。两组患儿治疗期间均无不良反应发生。结论维生素D联合寒喘祖帕颗粒治疗可以明显升高BA伴MP患儿的IL-12水平,降低MPO水平,提高临床疗效。Objective To explore the effects of vitamin D combined with Hanchuan Zupa granules on levels of serum interleukin-12(IL-12) and myeloperoxidase(MPO) in children with bronchial asthma(BA) and mycoplasma pneumoniae pneumonia(MPP).Methods A total of 82 children with BA and MPP confirmed in the hospital were enrolled between March 2019 and March 2021.They were divided into observation group and control group by simple random grouping method,41 cases in each group.The control group was treated with vitamin D,while observation group was treated with vitamin D and Hanchuan Zupa granules.All patients were continuously treated for 4 weeks.Before and after treatment,levels of serum IL-12,IgE and C-reactive protein(CRP) were measured by enzyme-linked immunosorbent assay.MPO content in neutrophil cytoplasm was detected by flow cytometry.The forced expiratory volume in 1 second(FEV1) and forced vital capacity(PVC) were detected by lung function detector.And FEV1/FVC ratio was calculated.The occurrence of adverse reactions during treatment in both groups was recorded.Results The response rate of treatment in observation group was significantly higher than that in control group(90.24% vs 73.17%)(P<0.05).After treatment,IL-12 level,FVC,FEV1 and FEV1/FVC in observation group were(227.53±81.56) pg/L,(3.29±0.26) L,(2.78±0.42) L and(76.54±5.93) %,significantly higher than those in control group [(186.72±86.92) pg/L,(2.93±0.31) L,(2.26±0.52) L,(72.88±4.53) %](P<0.05),while levels of serum IgE and MPO were(8.57±1.59) U/mL and(653.27±46.92) U/L,significantly lower than those in control group [(10.62±2.10) U/mL,(702.65±56.21) U/L](P<0.05).There were no adverse reactions during treatment in two groups.Conclusion Vitamin D combined with Hanchuan Zupa granules can significantly increase IL-12 level,decrease MPO level and improve clinical curative effect in children with BA and MPP.
