肤痔清软膏联合左西替利嗪治疗慢性肛周湿疹的临床研究

摘　　要：目的观察肤痔清软膏联合口服盐酸左西替利嗪片治疗慢性肛周湿疹的临床疗效。方法选择天津市宁河区医院在2018年3月—2020年6月收治的102例慢性肛周湿疹患者,随机分为对照组(51例)和治疗组(51例)。对照组患者睡前口服盐酸左西替利嗪片,5 mg/次,1次/d。治疗组在对照组基础上患处涂抹肤痔清软膏,早晚各1次。两组连续治疗12周。观察两组患者临床疗效,比较治疗前后两组患者炎症因子水平、肛周瘙痒程度、皮损面积及形态、生活质量指数和复发率。结果治疗后,治疗组患者临床有效率明显高于对照组(P<0.05)。治疗后,两组患者肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)明显下降,而γ干扰素(IFN-γ)明显升高(P<0.05),且治疗组IL-6、TNF-α和IFN-γ明显好于对照组(P<0.05)。治疗后,两组患者皮损面积及形态、肛周瘙痒程度、生活质量指数评分明显下降(P<0.05),且治疗组患者显著低于对照组(P<0.05)。治疗后,治疗组3、6个月的复发率明显低于对照组(P<0.05)。结论肤痔清软膏联合口服盐酸左西替利嗪片治疗慢性肛周湿疹,可有效改善其临床症状,减少复发率,降低炎症因子水平,疗效可靠。Objective To observe the efficacy of Fuzhiqing Ointment combined with levocetirizine in treatment of chronic perianal eczema. Methods Patients(102 cases) with chronic perianal eczema in Ninghe District Hospital from March 2018 to June 2020 were randomly divided into control(51 cases) and treatment(51 cases) group. Patients in the control group were po administered with Levocetirizine hydrochloride tablets before bed, 5 mg/time, once daily. Patients in the treatment group were administered with Fuzhiqing Ointment on affected area on the basis of the control group, twice daily. Patients in two groups were treated for 12 weeks.After treatment, the clinical evaluation was evaluated, the levels of inflammatory factors, perianal pruritus degree, area and shape of lesion, and quality of life index, and recurrence rate in two groups before and after treatment were compared. Results After treatment,the clinical effective rate of the treatment group was significantly higher than that of the control group(P < 0.05). After treatment,tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) were significantly decreased, while interferon-γ(IFN-γ) were significantly increased(P < 0.05), and IL-6, TNF-α and IFN-γ in the treatment group were significantly better than those in the control group(P <0.05). After treatment, the area and shape of skin lesions, the degree of perianal pruritus, and the scores of quality of life index were significantly decreased in two groups(P < 0.05), and which were significantly lower in the treatment group(P < 0.05). After treatment,the 3-month recurrence rate and 6-month recurrence rate in the treatment group were significantly lower than those in the control group(P < 0.05). Conclusion Fuzhiqing Ointment combined with levocetirizine in treatment of chronic perianal eczema can effectively improve the clinical symptoms, reduce the recurrence rateand level of inflammatory factors with reliable curative effect.
