200例高尿酸血症患者血清炎性因子水平与其HOMA-IR指数的关联性分析

摘　　要：目的探讨高尿酸血症患者血清炎性因子水平与其胰岛素抵抗(HOMA-IR)指数的关联性。方法选取我院2018年3月-2020年2月高尿酸血症患者200例为高尿酸血症组,同时选择同期健康体检者75例为对照组,统计比较两组空腹血糖(FPG)、空腹胰岛素(FINS)、超敏-C反应蛋白(hs-CRP)、白介素-18(IL-18)水平及HOMA-IR指数,分析血清hs-CRP、IL-18水平与其HOMA-IR指数的关联性。结果高尿酸血症组血清hs-CRP、IL-18水平高于对照组(P<0.05);高尿酸血症组FINS水平及HOMA-IR指数高于对照组(P<0.05);经Perason分析可知,血清hs-CRP、IL-18水平与HOMA-IR指数存在明显正相关关系(P<0.05);高尿酸血症患者治疗后血清hs-CRP、IL-18水平显著低于治疗前(P<0.05)。结论高尿酸血症患者血清hs-CRP、IL-18水平与HOMA-IR指数存在明显正相关关系,动态监测血清hs-CRP、IL-18水平,有助于临床判断疾病进展,同时为治疗效果评估提供数据支持。Objective To investigate the relationship between serum inflammatory factor levels and insulin resistance(HOMA-IR)index in patients with hyperuricemia.Methods 200 patients with hyperuricemia in our hospital from March 2018 to February 2020 were selected as the hyperuricemia group.According to whether there were clinical manifestations of gout or not, symptomatic patients were divided into group A(n=104),and asymptomatic patients were divided into group B(n=96).At the same time, 75 cases of healthy physical examination during the same period were selected as the control group.The fasting blood glucose(FPG),fasting insulin(FINS),high-sensitivity-C-reactive protein(hs-CRP),interleukin-18(IL-18)levels and HOMA-IR index were statistically compared between the two groups.Results The levels of serum hs-CRP and IL-18 in the hyperuricemia group were higher than those in the control group(P<0.05);the FINS level and HOMA-IR index of the hyperuricemia group were higher than those of the control group(P<0.05);Perason analysis showed that there was a significant positive correlation between serum hs-CRP and IL-18 levels and HOMA-IR index(P<0.05);serum hs-CRP and IL-18 levels in patients with hyperuricemia after treatment were significantly lower than before treatment(P<0.05).Conclusion The serum hs-CRP and IL-18 levels of patients with hyperuricemia are significantly positively correlated with HOMA-IR index.Dynamic monitoring of serum hs-CRP and IL-18 levels is helpful for clinical judgment of disease progression and provides data support for the evaluation of treatment effects.
