摘　　要：目的观察泡型肝包虫病患者肝组织中Smad2、Smad5、Smad7蛋白表达, 探讨各蛋白亚型在泡型肝包虫病肝纤维化进程中的作用、表达量的变化及其关系。方法收集20例泡型肝包虫病患者术中肝组织标本, 根据泡性肝包虫病患者病灶液化范围及手术切除标准, 每例分为A组(距离病灶0.5 cm内)、B组(距离病灶0.5~1.5 cm之间)、C组(距离病灶1.5 cm以外的肝组织)。对标本行苏木精-伊红(HE)及Masson染色, 检测各组织纤维化, 同时利用蛋白质印迹法(Western blot)技术检测Smad2、Smad5、Smad7蛋白表达量情况。通过相关性分析Smad2、Smad5、Smad7蛋白与HF程度之间的内在关系以及各蛋白之间的关系。结果在20例患者标本中, 病灶旁A组和B组组织中均存在1级(n=2、14)、2级肝纤维化(n=18、6), 未发现3级肝纤维化;且病灶旁组织越靠近病灶, 纤维化程度越高, 差异有统计学意义(χ^(2)=15.000, P<0.01)。Western blot检测发现, 同一组织中, 越靠近病灶, Smad2、Smad5蛋白表达量越高(F=48.336、75.988), Smad7蛋白表达量越低(F=44.202), 差异有统计学意义(P<0.01), A组Smad2指标高于B、C组(A组1.18±0.19比B组1.02±0.25比C组0.60±0.13, F=48.336, P<0.01);A组smad5指标高于B、C组(A组0.99±0.13比B组0.76±0.18比C组0.41±0.12, F=75.988, P<0.01);A组smad7指标低于B、C组(A组0.32±0.07比B组0.46±0.13比C组0.60±0.09, F=44.202, P<0.01)。不同组织中, 随着纤维化分级越高, Smad2、Smad5表达量越高(F=104.437、128.238), Smad7蛋白表达量越低(F=92.626), 差异有统计学意义(P<0.05), 2级肝纤维化组中Smad2指标高于1级肝纤维化组高于0级肝纤维化组(HF-2组1.24±0.16比HF-1组0.89±0.16比HF-0组0.60±0.13, F=104.437, P<0.05);2级肝纤维化组中Smad5指标高于1级肝纤维化、0级肝纤维化组(HF-2组0.72±0.28比HF-1组0.68±0.10比HF-0组0.41±0.12, F=128.238, P<0.05);2级肝纤维化组中Smad7指标低于1级肝纤维化、0级肝纤维化组(HF-2组0.31±Objective To observe the protein expression of Smad2,Smad5 and Smad7 in liver tissues of patients with hepatic alveolar echinococcosis,and analyze the role of each protein subtype in the progression of hepatic fibrosis in hepatic alveolar echinococcosis,the change of expression levels and their potential relationship.Methods Intraoperative liver tissue samples were collected from 20 patients with hepatic alveolar echinococcosis.According to the scope of lesion liquefaction and surgical resection criteria in patients with hepatic echinococcosis,the lesion in each patients was divided into group A(within 0.5 cm of the lesion),0.5 group B(within 0.5-1.5 cm of the lesion),and group C(>1.5 cm away from the lesion).The specimens were stained with HE and Masson to detect the fibrosis of each tissue,and the protein expression of Smad2,Smad5 and Smad7 was detected by Western blotting.Through correlation analysis,the intrinsic relationship between Smad2,Smad5 and Smad7 proteins and hepatic fibrosis level as well as the correlation between each protein were analyzed.Results In the samples of 20 patients,grade-1(n=2,14)and grade-2 liver fibrosis(n=18,6)was found in the group A and group B,and no grade-3 liver fibrosis was found.Moreover,the closer the adjacent tissues were to the lesion,the higher the degree of fibrosis was,which was statistically significant(χ^(2)=15.000,P<0.01).Western blotting showed that in the same tissue,the closer to the lesion,the higher the protein expression levels of Smad2 and Smad5(F=48.336,75.988)and the lower the protein expression levels of Smad7(F=44.202)were statistically significant(P<0.01).The index of SMAD2 in group A was higher than that in group B and group C(group A:1.18±0.19;group B:1.02±0.25;group C:0.60±0.13,F=48.336,P<0.01);Smad5 index in group A was higher than that in group B and group C(group A:0.99±0.13;group B:0.76±0.18;group C:0.41±0.12,F=75.988,P<0.01);Smad7 index in group A was lower than that in group B and group C(group A:0.32±0.07;group B:0.46±0.13;group C:0.60�
