摘  要:目的分析非小细胞肺癌(NSCLC)患者外周血自然杀伤(NK)细胞受体的表达及与肿瘤标志物的关系。方法收集122例NSCLC患者作为NSCLC组,选取同期46例健康体检者作为对照组,采集两组研究对象空腹外周静脉血,采用流式细胞仪检测外周血NK细胞受体[自然杀伤细胞2族成员D(NKG2D)、自然杀伤细胞活化性受体p30(NKp30)、自然杀伤细胞活化性受体p44(NKp44)、自然杀伤细胞2族成员A(NKG2A)、CD158a、CD158b]表达,采用酶联免疫吸附法检测血清肿瘤标志物[癌胚抗原(CEA)、细胞角质蛋白19片段抗原21-1(CYFRA21-1)、神经元特异性烯醇化酶(NSE)]水平,并采用Pearson相关分析法分析NSCLC患者外周血NK细胞受体表达与肿瘤标志物的关系。结果NSCLC组患者NK细胞活化性受体NKG2D、NKp30、NKp44表达水平均明显低于对照组,抑制性受体NKG2A、CD158a、CD158b表达水平均明显高于对照组,差异均有统计学意义(P﹤0.01)。Ⅲ~Ⅳ期NSCLC患者NK细胞活化性受体NKG2D、NKp30、NKp44表达水平均明显低于Ⅰ~Ⅱ期患者,抑制性受体NKG2A、CD158a、CD158b表达水平均明显高于Ⅰ~Ⅱ期患者,差异均有统计学意义(P﹤0.01)。NSCLC组患者血清肿瘤标志物CEA、CYFRA21-1、NSE水平均明显高于对照组,差异均有统计学意义(P﹤0.01)。Pearson相关性分析显示,NSCLC患者抑制性受体NKG2A表达与CEA水平呈正相关(P﹤0.05)。结论NSCLC患者外周血NK细胞活化性受体表达降低,抑制性受体表达升高,其中活化性受体表达与肿瘤标志物无相关性,抑制性受体NKG2A表达与肿瘤标志物CEA呈正相关。Objective To analyze the expression of natural killer(NK) cell receptors in peripheral blood of patients with non-small cell lung cancer(NSCLC) and their relationship with tumor markers. Method A total of 122 NSCLC patients were collected as the NSCLC group, and 46 healthy subjects received during the same period were selected as the control group. Fasting peripheral venous blood was collected from two groups of subjects, and flow cytometry was used to detect peripheral blood NK cell receptors [natural killer group 2 member D(NKG2D), natural killer p30(NKp30), natural killer p44(NKp44), natural killer group 2 member A(NKG2A), CD158a, CD158b] expression, serum tumor markers[carcinoembryonic antigen(CEA), cyto-keratin 19 fragment antigen 21-1(CYFRA21-1), neuron specific enolase(NSE)]levels were detected by enzyme-linked immunosorbent assay, and Pearson correlation analysis was used to analyze the relationship between tumor markers and the expression of NK cell receptors in peripheral blood from NSCLC patients.Result The expression levels of activating NK cell receptors NKG2D, NKp30, and NKp44 in the NSCLC group were significantly lower than those in the control group, and the expression levels of inhibitory receptors NKG2A, CD158a,and CD158b were significantly higher than those in the control group, and the differences were statistically significant(P<0.01). The expression levels of activating NK cell receptors NKG2D, NKp30, and NKp44 in patients with stage Ⅲ-Ⅳ NSCLC were significantly lower than those in patients with stage Ⅰ-Ⅱ, and the expression levels of inhibitory receptors NKG2A, CD158a, and CD158b were significantly higher than those in patients with stage Ⅰ-Ⅱ, the differences were statistically significant(P<0.01). The levels of serum tumor markers CEA, CYFRA21-1, and NSE in the NSCLC group were significantly higher than those in the control group, and the differences were statistically significant(P<0.01).
