摘　　要：目的探讨用药偏差管理在老年糖尿病患者医院-家庭过渡期的应用效果。方法选取四川省某三级甲等医院2020年7月至2021年6月收治的老年糖尿病患者102例,按随机数字表法分为对照组和试验组,各51例。试验组脱落1例,对照组脱落5例,均予以剔除。两组患者均实施内分泌科常规出院健康教育和用药指导;试验组在此基础上实施由护士主导的基于药物重整流程的用药偏差管理模式,内容包括收集、查对、修正、沟通。比较两组患者出院后的用药偏差发生率,并评估用药依从性。结果试验组患者出院后2个月的用药偏差发生率为18.00%,显著低于对照组的41.30%(P<0.05);试验组患者的用药依从性得分为(7.10±1.22)分,显著高于对照组的(6.50±1.28)分(P<0.05)。结论由护士主导的基于药物重整流程的用药偏差管理模式,可降低老年糖尿病患者在医院-家庭过渡期的用药偏差发生率,提高用药依从性。Objective To investigate the applied effect of medication discrepancies management in the hospital-family transition period of elderly patients with diabetes.Methods A total of 102 elderly patients with diabetes admitted to a three grade A hospital in Sichuan from July 2020 to June 2021 were selected and divided into the control group and the test group according to the random number table method,with 51 cases in each group.One case in the test group dropped out and five cases in the control group dropped out,all of which were excluded.Both groups were carried out routine discharge health education and medication guidance in the Department of Endocrinology,on this basis,the test group was carried out the medication discrepancies management mode led by nurses based on the medication reconciliation(Med Rec)process,including collection,checking,correction and communication.The incidences of medication discrepancies after discharge were compared between the two groups,and the medication compliance was evaluated.Results The incidence of medication discrepancies two months after discharge in the test group was 18.00%,which was significantly lower than 41.30%in the control group(P<0.05).The score of medication compliance in the test group was(7.10±1.22)points,which was significantly higher than(6.50±1.28)points in the control group(P<0.05).Conclusion The medication discrepancies management mode led by nurses based on the Med Rec process can reduce the incidence of medication discrepancies and improve medication compliance in elderly patients with diabetes during the hospital-family transition period.
