益肾坚骨汤辅助PFNA内固定术治疗股骨粗隆间骨折的效果及对PⅠNP水平的影响

摘　　要：目的分析益肾坚骨汤辅助股骨近端防旋髓内钉(PFNA)内固定术治疗股骨粗隆间骨折的效果及对Ⅰ型前胶原氨基端前肽(PⅠNP)水平的影响。方法98例股骨粗隆间骨折患者,按照随机数字表法分为对照组及研究组,每组49例。对照组接受PFNA内固定术治疗,研究组在PFNA内固定术基础上给予益肾坚骨汤辅助治疗。比较两组患者的疼痛程度、髋关节功能、PⅠNP水平、骨折愈合时间。结果术后1、3个月,研究组患者的疼痛数字评分法(NRS)评分分别为(4.32±1.27)、(2.05±0.78)分,均低于对照组的(4.91±1.35)、(3.14±0.92)分,差异具有统计学意义(P<0.05)。术后1、3个月,研究组患者的Harris评分分别为(71.38±4.65)、(82.79±3.36)分,均高于对照组的(60.19±5.07)、(80.63±3.52)分,差异具有统计学意义(P<0.05)。术后1、3个月,研究组患者的PⅠNP水平分别为(24.37±3.12)、(18.43±2.98)ng/ml,均低于对照组的(28.16±3.64)、(20.05±3.19)ng/ml,差异具有统计学意义(P<0.05)。研究组患者的骨折愈合时间(12.04±2.17)周短于对照组的(14.58±2.43)周,差异具有统计学意义(P<0.05)。结论益肾坚骨汤辅助PFNA内固定术治疗可以减轻股骨粗隆间骨折患者的疼痛程度,促进骨折愈合,并调节骨代谢指标及改善髋关节功能,值得临床重视。Objective To analyze the effect of Yishen Jiangu Decoction assisted proximal femoral nail antirotation(PFNA)internal fixation in the treatment of intertrochanteric fracture of femur and its influence on N-terminal propeptide of typeⅠprocollagen(PINP)level.Methods A total of 98 patients with intertrochanteric fracture of femur were divided into control group and research group according to the random numerical table,with 49 cases in each group.The control group received PFNA internal fixation,and the research group received Yishen Jiangu Decoction on the basis of PFNA internal fixation.The pain degree,hip function,PⅠNP level and fracture healing time were compared between the two groups.Results At 1 and 3 months after surgery,the numerical rating scale(NRS)scores in the research group were(4.32±1.27)and(2.05±0.78)points,which were lower than(4.91±1.35)and(3.14±0.92)points in the control group,and the difference was statistically significant(P<0.05).At 1 and 3 months after surgery,the Harris scores in the research group were(71.38±4.65)and(82.79±3.36)points,which were higher than(60.19±5.07)and(80.63±3.52)points in the control group,and the differences were statistically significant(P<0.05).At 1 and 3 months after surgery,the PINP levels in the research group were(24.37±3.12)and(18.43±2.98)ng/ml,which were lower than(28.16±3.64)and(20.05±3.19)ng/ml in the control group,and the difference was statistically significant(P<0.05).The fracture healing time in the research group was(12.04±2.17)weeks,which was shorter than(14.58±2.43)weeks in the control group,and the difference was statistically significant(P<0.05).Conclusion Yishen Jiangu Decoction assisted PFNA internal fixation can reduce the pain of patients with intertrochanteric fracture of femur,promote fracture healing,regulate bone metabolism index and improve hip joint function,which is worthy of clinical attention.
