益气活血方结合穴位贴敷治疗胃癌前病变对COX-2、VEGF表达的影响

摘　　要：目的探讨益气活血方结合穴位贴敷治疗胃癌前病变对环氧合酶-2(COX-2)、血管内皮生长因子(VEGF)表达的影响。方法92例胃癌前病变患者,随机分为对照组及观察组,每组46例。对照组使用叶酸治疗,观察组在对照组基础上使用益气活血方结合穴位贴敷治疗。比较两组临床疗效,治疗前后中医证候积分及COX-2、VEGF表达水平。结果观察组治疗总有效率93.48%高于对照组的71.74%,差异具有统计学意义(P<0.05)。治疗后,观察组的胃痛、痞满、嗳气、纳差、乏力积分均低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的COX-2(30.25±7.96)ng/ml、VEGF(96.41±23.68)ng/L均低于对照组的(61.64±9.18)ng/ml、(132.58±30.19)ng/L,差异具有统计学意义(P<0.05)。结论益气活血方结合穴位贴敷治疗胃癌前病变的效果显著,能有效缓解中医证候积分,降低COX-2、VEGF表达,有助于降低癌变几率、减慢癌变进程。Objective To discuss the effect of Yiqi Huoxue recipe combined with acupoint application on the expression of cyclooxygenase-2(COX-2)and vascular endothelial growth factor(VEGF)in the treatment of precancerous lesions of gastric cancer.Methods A total of 92 patients with precancerous lesions of gastric cancer were randomly divided into control group and observation group,with 46 cases in each group.The control group was treated with folic acid,and the observation group was treated with Yiqi Huoxue recipe combined with acupoint application on the basis of the control group.The clinical efficacy,traditional Chinese medicine syndrome scores,COX-2 and VEGF expression levels before and after treatment were compared between the two groups.Results The total effective rate 93.48%of the observation group was higher than 71.74%of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of stomachache,fullness,belching,anorexia and fatigue in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the COX-2(30.25±7.96)ng/ml and VEGF(96.41±23.68)ng/L in the observation group were lower than(61.64±9.18)ng/ml and(132.58±30.19)ng/L in the control group,and the difference was statistically significant(P<0.05).Conclusion Yiqi Huoxue recipe combined with acupoint application is effective in the treatment of precancerous lesions of gastric cancer.It can effectively alleviate the syndrome score of traditional Chinese medicine,reduce the expression of COX-2 and VEGF,help to reduce the probability of canceration and slow down the process of canceration.
