自制恒温湿化吸氧装置在留置人工气道患者中的应用研究

摘  要:目的观察自制恒温湿化吸氧装置在留置人工气道患者中的应用效果。方法60例人工气道患者,按单双数分为观察组和对照组,每组30例。对照组按常规接受传统的室温灭菌注射用水湿化瓶导管吸氧(普通湿化瓶吸氧、微泵湿化法),观察组采用自制恒温湿化吸氧装置联合改良导管吸氧法。比较两组呼吸频率、动脉血氧分压(PaO_(2))、血氧饱和度(SpO_(2))以及痰液粘稠度、痰痂形成、肺部感染情况。结果治疗后,观察组患者的呼吸频率(16.05±0.14)次/min低于对照组的(17.96±1.71)次/min,PaO_(2)(93.30±2.09)mm Hg(1 mm Hg=0.133 kPa)、SpO_(2)(95.58±0.73)%均高于对照组的(88.17±3.54)mm Hg、(92.42±0.91)%,差异具有统计学意义(P<0.05)。观察组痰液粘稠度分级情况优于对照组,痰痂形成率3.33%、肺部感染率13.33%均低于对照组的23.33%、46.67%,差异均具有统计学意义(P<0.05)。结论在人工气道患者中,以自制恒温湿化吸氧装置联合改良导管吸氧法,可明显的平稳呼吸,改善血氧指标,减轻痰液粘稠度,降低痰痂形成、肺部感染,效果理想。Objective To observe the application effect of self-made constant temperature humidification device in patients with artificial airway.Methods A total of 60 patients with artificial airway were divided into observation group and control group according to odd and even numbers,with 30 cases in each group.The control group received conventional room temperature sterilized water for injection humidified bottle catheter oxygen inhalation(ordinary humidified bottle oxygen inhalation,micro-pump humidification method),and the observation group received the self-made constant temperature humidification device in combination with the improved catheter oxygen inhalation method.Both groups were compared in terms of respiratory rate,partial pressure of arterial oxygen(PaO_(2)),blood oxygen saturation(SpO_(2)),sputum viscosity,sputum crust formation,and pulmonary infection.Results After treatment,the respiratory rate(16.05±0.14)times/min of the observation group was lower than(17.96±1.71)times/min of the control group,and the PaO_(2)(93.30±2.09)mm Hg(1 mm Hg=0.133 kPa)and SpO_(2)(95.58±0.73)%were higher than(88.17±3.54)mm Hg and(92.42±0.91)%of the control group.All the differences were statistically significant(P<0.05).The sputum viscosity classification in the observation group was better than that in the control group,and the sputum crust formation rate 3.33%and pulmonary infection rate 13.33%were lower than 23.33%and 46.67%in the control group,and the differences were statistically significant(P<0.05).Conclusion Self-made constant temperature humidification device combined with improved catheter oxygen inhalation method has ideal effect on patients with artificial airway,and can significantly stabilize breathing,improve blood oxygen index,reduce sputum viscosity,reduce the formation of sputum scab and pulmonary infection.
