基于循证构建老年尿失禁患者出院准备护理方案

摘　　要：目的构建老年尿失禁患者出院准备护理方案,为提高出院指导质量提供参考依据。方法基于循证,检索中外数据库,获取与老年尿失禁出院准备相关文献并进行文献质量评价,结合临床实践,采用小组讨论方法制定老年尿失禁患者出院准备护理草案,通过2轮德尔菲专家函询对草案进行修订确定最终方案。结果2轮专家函询中,专家积极系数均为100%,专家权威系数为0.86、0.82,肯德尔和谐系数为0.29、0.34,形成包括排尿情况、疾病知识、心理状况、出院护理需求评估、日常生活方式5个方面共21个条目的老年尿失禁患者出院准备护理方案。结论老年尿失禁患者出院准备护理方案构建具有一定的科学性、可靠性,可为临床老年尿失禁的管理提供参考与借鉴。Objective To construct a nursing plan of discharge preparation for elderly patients with urinary incontinence and to provide reference for improving the quality of discharge guidance. Methods With evidence-based method, we retrieved the Chinese and foreign databases to collect the literature related to the discharge preparation for elderly patients with urinary incontinence and evaluate the quality of the literature. The draft of nursing plan of discharge preparation for elderly patients with urinary incontinence was formed by group discussion, and the draft was revised through two rounds of Delphi expert consultation. Results In the two rounds of expert consultation, the positive coefficient was 100%;expert authority coefficient 0.86 and 0.82, and the Kendall harmony coefficient 0.29 and 0.34 respectively. The final nursing plan for discharge preparation of elderly patients with urinary incontinence consisted of 21 items in 5 dimensions including urination, disease knowledge, psychological status, assessment of discharge nursing needs and daily lifestyle. Conclusion The constructed nursing plan of discharge preparation for elderly patients with urinary incontinence is scientific and reliable, which provides reference for the management of urinary incontinence in clinical practice.
