摘　　要：目的研究虹口区2010—2020年居民伤害死亡流行特征,为制定上海市虹口区有针对性的伤害防制措施提供依据。方法资料来源于上海市虹口区户籍居民的死因监测系统。采用粗死亡率、标化死亡率、年均变化百分比及潜在寿命损失年(PYLL)等指标进行描述。结果2010—2020年虹口区户籍居民伤害粗死亡率为42.50/10万,标化死亡率为19.96/10万,且随年份呈下降趋势(t=-5.1,P<0.001),前5位死因依次为跌倒/坠落、交通事故、自杀、窒息、溺水,其中跌倒/坠落随年份呈上升趋势(t=7.3,P<0.001),交通事故随年份呈下降趋势(t=2.9,P<0.05)。男性伤害死亡率为41.00/10万,女性为43.94/10万,男性和女性前三位伤害死因均为跌倒/坠落、交通事故、自杀;0~14岁组、15~39岁组、40~64岁组、65岁及以上组的首位死因分别为跌倒/坠落、自杀、交通事故、跌倒/坠落。伤害导致的PYLL为68915.25人年。寿命损失居前三位的为跌倒/坠落、交通事故、自杀。结论伤害威胁着虹口区户籍居民健康,并且有年龄、性别的差异,今后需结合人群特点采取相应的防制措施以降低死亡率。Objective To study the epidemiological characteristics of injury and death among residents in Hongkou District from 2010 to 2020 and to provide scientific basis for formulating injury prevention and control measures.Methods The data were collected from the monitoring of the cause of death of residents registered in the Hongkou District.Indicators including crude death rate,standardized death rate,average annual change percentage,and potential life lost years(PYLL)were used to describe.Results From 2010 to 2020,the crude death rate of Hongkou District household registration residents was 42.50/100000,and the standardized death rate was 19.96/100000,and it showed a downward trend with the year(t=-5.1,P<0.001).The top five causes of death were tumbles/falls,traffic accidents,suicide,suffocation,and drowning,among which tumbles/falls show an upward trend with the year(t=7.3,P<0.001),but traffic accidents showed a downward trend with the year(t=2.9,P<0.05).The death rate for injuries was 41.00 per 100000 for men and 43.94 per 100000 for women.The top three causes of death for injuries were the same for both men and women,namely tumbles/falls,traffic accidents,and suicide.The leading causes of death in the 0-14 years old group,15-39 years old group,40-64 years old group,and 65 years old and above group were tumbles/falls,suicide,traffic accidents,and tumbles/falls,respectively.The PYLL caused by the injury was 68915.25 person-years.The top three life losses were tumbles/falls,traffic accidents,and suicide.Conclusion Injuries threaten the health of registered residents in Hongkou District,with differences in age and gender.In the future,it is necessary to take preventive measures based on the characteristics of the population to reduce the mortality rate.
