摘  要:间充质干细胞来源的外泌体在心肌缺血再灌注损伤后能显著抑制细胞凋亡、调节细胞自噬,但具体机制尚不明确。近期研究发现miR-155介导PI3K/mTOR信号通路的激活在诱导细胞自噬发生方面发挥了调控作用。前期研究发现中药复方芪参益气滴丸联合外泌体能显著改善心肌缺血再灌注损伤及调控巨噬细胞表型、改善炎性微环境,且细胞实验证实芪参益气滴丸可以激活PI3K/mTOR信号通路从而改善心肌缺血。故提出假说芪参益气滴丸调控外泌体miR-155靶向介导PI3K/mTOR信号通路调节缺血区巨噬细胞表型,起到调节缺血再灌注后局部炎症、改善心功能的作用。文章从通过对心肌缺血后的炎症反应与巨噬细胞相关性,及其与芪参益气滴丸调控外泌体miR-155靶向介导PI3K/mTOR信号通路调节缺血区巨噬细胞表型的可能机制进行综述,为心血管研究领域提供一个新的研究思路。The exosomes derived from mesenchymal stem cells can significantly inhibit apoptosis and regulate autophagy after myocardial ischemia-reperfusion injury,but the specific mechanism is not clear.Recently,it was found that mi R-155 mediated activation of PI3 K/m TOR signaling pathway plays a regulatory role in inducing autophagy.In our previous study,we found that Qishen Yiqi Dropping Pill(芪参益气滴丸)combined with exocrine energy can significantly improve myocardial ischemia-reperfusion injury,regulate macrophage phenotype,and improve inflammatory microenvironment,and the cell reality proved that Qishen Yiqi Dropping Pill can activate PI3 K/m TOR signal pathway to improve myocardial ischemia.Therefore,the author proposes the hypothesis that Qishen Yiqi Dropping Pill regulates exocrine mi R-155 targeting and mediates PI3 K/m TOR signal pathway to regulate the phenotype of macrophages in the ischemic area,which plays a role in regulating local inflammation and improving heart function after ischemia-reperfusion.This article reviews the possible mechanism of PI3 K/m TOR signal pathway regulating the phenotype of macrophages in the ischemic area through the correlation between inflammatory response and macrophages after myocardial ischemia,and the regulation of exocrine miR-155 by Qishen Yiqi Dropping Pill,which provides a new research idea for the field of cardiovascular research.
