摘  要:糖尿病肾病(diabetic kidney disease,DKD)核心病机观在于络虚邪瘀,发生以"肾络"为要。治疗当以通补络脉法为总体治则,以达"通"之目的。结合现代中医药从络论治DKD的研究进展,提出通补络脉法扭转DKD络虚邪瘀核心病机的现代分子学机制乃调节免疫炎症分子、调控糖脂代谢异常和细胞自噬、改善血液微循环障碍、抗氧化应激反应和抑制细胞焦亡,达到通利"络热""络痰""络瘀""络毒"功效,从而延缓DKD发生发展进程。The core pathogenesis of diabetic kidney disease(DKD)is collateral deficiency and evil stasis,the occurrence of“kidney collaterals”is important. It is effective to take dredging and tonifying collaterals method as the overall principle in treating DKD,in order to achieve the purpose of dredging.Combined with the research progress of modern traditional Chinese medicine in treatment of DKD from collaterals,it is proposed that the modern molecular mechanism of dredging and tonifying collaterals method for reversing the core pathogenesis of diabetic kidney disease with collateral deficiency and evil stasis is to regulate immune inflammatory molecules,regulate abnormal glucose and lipid metabolism and cell autophagy,improve blood microcirculation disorder,antioxidant stress response and inhibit pyroptosis,in order to achieve the effects of dredging“collaterals heat”“collaterals phlegm”“collaterals stasis”“collaterals toxin”,and delay the occurrence and development process of DKD.
